An exciting opportunity to enhance your career is available - join our integrated team in the Emergency Department (ED) at Charing Cross Hospital. Our Therapy department provides opportunity for enthusiastic and highly motivated therapists to expand their skills in admission avoidance, complex discharge planning, and frailty. We pride ourselves in providing a high quality of care for our patients utilising the Comprehensive Geriatric Assessment (CGA) and working along-side our Frailty Clinical Nurse Specialists.

We are seeking a dynamic Therapist who thrives in a fast paced environment where no two days are the same. You will have the opportunity to assess and treat a wide range of patient cohorts; frailty, falls, dementia, delirium, musculoskeletal injuries, neurological conditions and other long term conditions.

Our team consists of occupational therapists and physiotherapists; we are an interdisciplinary team encouraging skill sharing to empower therapists to be holistic and autonomous practitioners whilst also appreciating profession specific competencies. We are an admission avoidance focussed service, however also appreciate the importance of early therapy intervention for those requiring hospital admission. This is a role which sees a high quality, positive impact on its service users, reducing length of stay and encouraging patient involvement all stages of decision making.

As a team we champion Quality Improvement and you will have opportunities to be involved in Quality Improvement initiatives as well as multidisciplinary team education as part of this forward thinking clinical team. We embed the ethos of the campaign - NHS England's 'AHPs into Action' to pioneer positive change. We also strive to deliver evidence based practise under the Royal College of Emergency Medicine guidelines to inform our projects.

The Therapy Team in the Emergency Department also work as an integral MDT member of OPRAC team (Older Persons Rapid Access Clinic) where we complete therapy specific outcome measures and provide treatments such as equipment provision, home exercise programmes, advice, education, and refer/signpost to appropriate community services. This is a proactive in reach clinic to better health outcomes and improve quality of life for our elderly population.

You will be expected to be able to work in collaboration with the OPAL (Older Persons Assessment and Liaison) team, which is multidisciplinary team working on the Acute Medical wards. This is a gold standard service which aims to complete CGAs to formulate holistic person centred treatments and discharge plans.

Our service operates from 8am-6pm Monday to Friday with varied shift patterns of hours. You will also be a part of the weekend service in OPAL and ED, prioritising patients medically optimised for discharge where you will then be allocated rest days.

We are an improvement and education focussed team and value the career progression and clinical development of our team. You will have access to a robust in-service training programme, clinical supervision and wider training and learning opportunities.

Main duties of the job

To provide high quality therapy management to patients at Imperial College Healthcare NHS Trust.

To perform advanced interdisciplinary therapy assessment of patients with diverse presentations and complex physical and psychological conditions,

To comprehensively assess and develop individualised treatment plans for patients entering the hospital via Accident and Emergency, cross covering where the service requires

The job holder is required to work at weekends as part of their contracted hours of work, (specific rota details are held locally within departments).

Person Specification

Education/ Qualifications

Essential

Degree (or equivalent) in Physiotherapy or Occupational Therapy State registration with the Health Professions Council. Able to demonstrate specialist learning from previous physiotherapy rotations, formal post-graduate level learning and informal CPD (both general and specific to clinical fields of speciality).

Desirable

Membership of the Chartered Society of Physiotherapy or Royal College of Occupational Therapy Membership of CSP/RCOT special interest groups relevant to speciality Clinical Educators Course

Experience

Essential

Recent substantial and sustained experience in relevant therapy specialities and clinical settings Experience of clinical education of staff, students or assistants. To have undertaken audit and/or research, either within area of speciality or as part of the MDT To have participated in service development projects. Experience of working as part of a multidisciplinary team Demonstrate an understanding of the use of evidence based practice in speciality

Desirable

Previous experience in the NHS as a qualified physiotherapist or Occupational Therapist Experience of on-call and weekend working in an acute hospital

Skills/Knowledge/ Abilities

Essential

Able to apply a range and depth of knowledge to this specialist area. Able to use knowledge of a range of treatment approaches in the specialist area Able to make sound clinical decisions based on knowledge/ experience/circumstances and utilise advanced clinical reasoning skills.
