Job summary

They will work alongside socialprescribing link workers and health and Wellbeing for Life to provide anall-encompassing approach to personalised care and enable people navigatethrough the health and care system.

The post holder will work with a diverserange of people from different cultural and social backgrounds. The ability towork confidently and effectively in a varied and sometimes challengingenvironment is essential.

The successful candidate will haveexcellent interpersonal and communication skills, and be organised, patient andempathetic. They will have experience of working in health, social care orother support roles including direct contact with people, families, or carers.

Main duties of the job

Proactively identify and work with people, who engage with the practiceirregularly and those who fail to engage with smear programmes, childvaccinations etc.

Work people, including the frail/elderly and those with long-termconditions, who regularly do not attend appointments.

Develop relationships with the hard- to- reach groups and understand howthe practice can adapt to better understand and meet their needs.

Help people to manage their needs through answering queries, making, andmanaging appointments, and ensuring that people have good quality written orverbal information to help them make choices about their care.

Work closely with GPs and practice teams, making sure that appropriateclinical support is made available to people.

Provide coordination and navigation for people and their carers acrosshealth and care services, working closely with social prescribing link workers,health and wellbeing coaches, and other primary care professionals; helping toensure patients receive a joined-up service and the most appropriate support.

Signpost patients to the most suitable external organisations who couldsupport with the issues that were less directly health- related, such ascitizens advice, wellbeing for life, exercise referral, community groups etc.

Attend multidisciplinary team meetings within the practice to ensureappropriate follow up for patients occur

Identifyunpaid carers and help them access services to support them.

Job responsibilities

We are looking to recruit to the post of Care Coordinator, to work at our Wingate Practice

The Care Coordinator would also signpost patients to the most suitable external organisations who could support with issues that were less directly health- related, such as citizens advice, wellbeing for life, exercise referral, community groups etc.

They will work alongside social prescribing link workers and health and Wellbeing for Life to provide an all-encompassing approach to personalised care and enable people navigate through the health and care system.

The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied and sometimes challenging environment is essential.

The successful candidate will have excellent interpersonal and communication skills, and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles including direct contact with people, families, or carers.

Care coordinators play an important role within a practice to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

Care coordinators will attend multidisciplinary team meetings within the practice to ensure appropriate follow up for patients occur.

They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families, and carers with high quality support.

Please note that the role of a care coordinator is not a clinical role.

Qualifications

Essential

Demonstrable commitment to professional and personal development Ability to use Microsoft Office applications Word, Excel, PowerPoint, Outlook

NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards

Experience

Essential

Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity) Experience of working within multi- professional team environments Experience of supporting people, their families and carers in a related role

Desirable

Experience of working directly in a care coordinator role, adult health and social care, learning support or public health /health improvement Experience of working in General Practice or in primary care. Experience of using General Practice IT systems in particular System 1. Experience of working with elderly or vulnerable people, complying with best practice and relevant legislation
