Job summary
We are looking for anengaged, positive, friendly, hard-working, and resilient individual to join oursalaried GP team at the Elms Medical Practice and provide maternity cover fromJune 2024.
The post is for 4 sessionsa week (Wednesday and Friday). We are happy to be flexible where we can aroundworking days.
Main duties of the job
Provide medical services for thepractice population to a high standard, including day-to-day activities such assurgery consultations, telephone consultations, home visits and appropriate administrationto ensure accurate records, patient continuity and quality of care.
Comply with the requirements ofthe practices General Medical Services/Personal Medical Services (GMS/PMS/APMS)contract, Enhanced Services contracts, educational/training delivery contractsand private contracts.
Adhere to the principles of goodmedical practice.
About us
We serve a patientpopulation of 9885 patients living in Chester and surrounding areas operatingout of 2 newly built sites. Our main surgery is in Chester City Centre and ourbranch surgery is in Blacon. We are a high achieving and busy practice with 3 partners,4 salaried GPs, ANP, 2 practice nurses and a HCA. Our clinical team aresupported by a large admin team which includes a GPA and Pharmacy Technician.As part of the Chester Central PCN we have a larger multi-disciplinary teamconsisting of paramedics, clinical pharmacists, Physiotherapists, Counsellors,Dietitian and Health and well-being coaches who are organised by our CareCo-ordinator.
We are a training practiceso have experience in helping to guide newer members of the profession throughthe early stages we also have a number of highly experienced colleagues so therole would be suitable for someone at any stage in their qualified GP career.
Job description
Job responsibilities
Provide high quality medicalservices to patients registered with the practice.
Provide a full range of servicesin line with the requirements of the GMS/PMS/APMS) Contract and EnhancedServices Contracts.
Refer patients as appropriateand in accordance with local procedures.
Undertake a fair share of dutiessuch as prescription signing, prescribing reviews, Pathology and hospitalletter management & duty doctor in accordance with the rota.
Undertake clinics, post-natalchecks, child developmental checks as agreed or required.
Manage long-term conditions inaccordance with practice and national clinical guidelines.
Prescribe generically and inaccordance with the practice formulary.
Provide appropriate healthpromotion advice to all patients.
Ensure good clinical governanceis part of patient care and other practice activities.
Adhere to practice guidelines,policies, and procedures.
Practice evidence-basedmedicine.
Take ongoing responsibility forpatients with difficult and complex problems including consistentfollow-through.
Participate in audit and qualityimprovement processes.
Maintain clinical records(manual/electronic) in line with good practice and in accordance with practiceprocedures including appropriate coding as agreed in the practice.
Maintain administrative recordsin relation to GMS/PMS/APMS) contract, enhanced services contracts and QOFincluding coding.
