Job summary

Weare a friendly, forward looking and enthusiastic PMS practice of 18,500patients. The practice has easy access from the A19 and Durham. We are atraining practice and host GP Registrars and also support with supervision andmentoring of Foundation Doctors , Medical Students and various ARRS rolesincluding Physician Associates, Pharmacists, FCP and other inhouse developmentroles.

Main duties of the job

Asa key member of the Primary Care Team, the GP will manage a case load, dealingwith a wide range of presenting health care needs in a primary care setting.The post holder will work with clinical colleagues to ensure the higheststandards of care are provided for all registered and temporary patients. Thepost holder will also contribute to planning and decision making as a part ofthe wider practice team.

Job description

Job responsibilities

In general the post holder will be expected to undertake allthe normal duties and responsibilities associated with a GP working withinprimary care including;

• Makingprofessionally autonomous decisions in relation to presenting problems, whetherself-referred or referred internally within the practice

• Receivepatients with undifferentiated and undiagnosed problems and make assessments oftheir health care needs

• Consultwith patients in the surgery, at home visits and via telephone or on lineconsultations as agreed internally between the practice GP’s

• Undertakestriage calls, triage visits, checking and signing of repeat prescriptions anddealing with queries, paperwork and correspondence in a timely fashion asrequired.

• Screenpatients for disease risk factors and early signs of illness

• Inconsultation with patients and in line with current practice disease managementprotocols, develops care plans for health

• Providecounselling and health education as necessary

• Admit ordischarge patients to and from caseload and refer them to other care providersinternally and externally as appropriate and in line with practice protocols.

• Complywith all relevant clinical Governance and Practice policies and protocols.

• Assist inthe establishment of appropriate systems to manage common chronic medicalconditions.

• Completeclinically related administrative and non-clinical duties required for thedelivery of the service.

• Recordclear and contemporaneous consultation notes to agreed standards.

• Demonstratecommitment to lifelong learning, and audit and effectiveness to ensure evidencebased best practice.

• Provideactive input to the wider Primary healthcare team in the process of healthneeds research/ base line data collection/clinical audit.

• Compileand issue computer generated acute and repeat prescriptions, prescribing inaccordance with practice/DDES CCG prescribing formulary whenever this isclinically appropriate.

• Networkwith other agencies (both statutory and voluntary) and develop appropriate linksto further the work of the practice with the CCG and Federation

• Meetprofessional learning and CPD requirements.

• Developeffective relationships with clinical supervisor/mentors and other colleagues.

PRACTICE RESPONSIBILITIES

• Contributeto the evaluation/ audit and clinical standard setting with colleagues.

• Recorddata in patient records systems promptly and accurately and to agreedstandards.

• Ensureappropriate use of read codes.

• Participatein the collection and collation of statistics towards the preparation ofpractice reports, performance monitoring and audits.

• Participatein the auditing of practice activity as required.

• Contributeto the strategic management and development of the practice.

• Providereports to both NHS and non-NHS agencies as required.

• Responsiblefor health and safety of themselves and others and for reporting and activelyco-operating in Alliance and Practice Health and Safety Policies andprocedures.

• Processand action incoming patient and hospital correspondence.

• Attend inhouse meetings, significant event audit meetings and other meeting as deemednecessary.

• Ensurethat the practice complaints system is adhered to at all times.

• Awarenessand compliance with all relevant guidelines data protection,confidentiality, and health and safety.

• Maintaincurrent CPR and anaphylaxis certification and child/adult safeguarding trainingin line with local policies.

• Maintainmandatory training and revalidation certification
