Proactive Care - Enhanced Health in Care Homes- Learning Disability Team is for you This role is ideal for a person with a passion for utilising your skills and experience for supporting clinicians and clients.

We are looking for someone who has excellent holistic personalised care skills and able to support assessment and interventions to improve the physical health, mental health, and wellbeing of people with Learning disability with long-term living in the borough of Haringey.

The role is a great opportunity to work in partnership with other professionals within the team including: - Consultant Geriatricians, GPs, Pharmacists, Community Matrons, Trusted Assessors, Haringey learning disability team supported by a fantastic administration team.

You will be expected to embrace activities and tasks outside of traditional Nursing Associate role adopting shared competencies to be able to support the whole MDT.

You will be supported to develop skills and competences in frailty and would have the opportunity to stretch and develop your skills with the support of the team and with regular professional supervision.

Main duties of the job

·The Nursing Associate is a registered practitioner with the NMC and will act in accordance with the NMC Nursing Associate Standards of

·Proficiency and the NMC code.

·The post holder will be:

·Responsible for providing care and therapy to patients as delegated by the registered nurse or MDT Professionals to ensure a seamless service to patients.

·Allocated a caseload of patients by the registered nurse or MDT and will be expected to work independently, holistically assessing patients’ needs, planning and prioritising their workload and implementing evidence-based care, evaluating the outcomes of care under the minimal supervision of the registered nurse.

·Required to provide optimum quality of care to patients, offering support to the multi-professional team and will take responsibility for an allocated caseload.

 Key Results

 To provide and monitor safe, person-centred care to a designated group of patients, service users and families in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.

To support the registered nurse and contribute to the on-going assessment and care planning of patients and service users to improve and maintain their mental, physical, behavioural health and wellbeing.

To act in the best interests of patients and service users to promote and prevent ill health and contribute to their integrated care.

To monitor the condition and health needs of patients and service users on a continual basis in partnership with colleagues, families, and carers, referring to others for reassessment, when required

The Nursing Associate will actively contribute to an effective learning environment and support others in their learning.

Nursing Associates will recognise and adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

Demonstrate ability to deliver safe and evidenced based care, contribute to ongoing assessment recognising when it is necessary to refer and escalate to others for reassessment.

Provide compassionate, safe, and effective care and support to people in a range of care settings.

Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.

 Record all care delivered appropriately and contemporaneously. Documentation must be clear, concise, and legible. Any documentation must also be signed, stamped, and dated.

 Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating, or handing over responsibility for care

 Recognise and report any situations, behaviours or errors that could result in poor care outcomes

 Identify and report to a Registered Nurse clinical risk issues that impact on the patients /client’s health or social care needs.

NMC registered Nursing associate foundation qualification GCSE Grade A-C in Maths & English or skills leve 2 Maths & English or equivalent
