Job summary

In this role you will provide clinical leadership on medicines optimisation and quality improvement to GP/primary care network (PCN) pharmacy teams across the integrated care area (ICA). You will be engaging and dynamic in order to support integration with community pharmacy and provide clinical input for future commissioned community pharmacy services to the primary care team of the CIOSICB. A key aspect of this role is to ensure high-quality, cost-effective prescribing and medicines management across GP practices, prioritising the CIOSICB medicines optimisation agenda, the cost improvement programme and financial recovery plan. You will be adept at fostering positive relationships in order to work in partnership on various ICB and system functions including the population health management agenda and the national medicines optimisation opportunities and support integration with community pharmacy across the ICA and to collaborate with PCN community pharmacy clinical lead and community pharmacy Cornwall.

Main duties of the job

To take a lead in strategic and operational planning in order to deliver a high quality of pharmaceutical care and medicines optimisation to the CIOSICB population, taking account of local and national strategies.

To support the delivery of specific prescribing initiatives and strategies as part of the cost improvement programme and financial recovery plan and medicines optimisation agenda.

To ensure a consistent approach to medicines management across the system, to maximise benefit and minimise risk to patients from their medicines, through close liaison with other lead pharmacy colleagues.

To ensure high quality, evidence based, cost effective prescribing in line with the national institute for health and care excellence (NICE) and other national drivers.

To be responsible professional leadership to GP/PCN pharmacy teams across the integrated care area (ICA).

Knowledge and experience

Essential

oExperience of working in GP practices oUp to date knowledge of prescribing issues in primary and secondary care, community pharmacy and current health policy oDemonstrable experience of prescribing advisory role and able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions. oIn depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. oKnowledge of pharmacy contracts and drug tariff oAn understanding of law relating to controlled drugs and the supply of medicine

Qualifications
