Job summary
FIXED TERM CONTRACT UNTIL 31st MARCH 2025 IN THE FIRST INSTANCE, WITH POSSIBILITY TO EXTEND
This role will cover GP Surgeries within Nottingham City PCN 8, which incorporates Clifton and The Meadows areas of Nottingham.
ThePrimary Care Network Cancer Care Coordination service works from within GP surgeries to providehigh quality, holistic support to patients on the Cancer Pathway. Workingclosely with an primary care professionals and external services, ourdedicated Cancer Care Coordinators work tirelessly to address the widerdeterminants of health and wellbeing from within the health service. As well as removing barriers to engagement and improving health outcomes, the work of the Cancer Care Coordinators can provide lifechanging support for patients.
Main duties of the job
Conducting in person Cancer Care Reviews and liaising with a range of professionals to address the wider wellbeing needs of thepatient.
Casework to ensure these needs are met and make sure the patient is holistically supported, whether by yourself, the Cancer Nurse Specialist, the GP, or another service.
Interacting with professionals within the PCN to supporteach other in ensuring the best cancer journey for each patient.
There is alsoscope to be involved in community outreach and screening initiatives.

This service works closely with the Macmillan Beyond Diagnosis Service (also hosted by Self Help UK), providing community based support for those affected by cancer.

Supporting Patients
Work closely with GP surgeries to offer Cancer Care Reviews, Holistic Needs Assessments, and any other appropriate support to patients on the cancer pathway Understand processes in individual GP surgeries to ensure their patients receive care and support with the appropriate and agreed processes Co-Produce care and support plans with the patient to maximise their wellbeing Facilitate entry to appropriate services through referrals and signposting Work closely with external groups and organisations that can offer support to patients Work closely with Macmillan Beyond Diagnosis Service to ensure a sharing of knowledge, expertise, and mutual support to maximise outcomes for all beneficiaries Liaise with practice staff using appropriate processes to ensure best use of resources and effective partnership workingCancer Screening Uptake
Work with Self Help UK Manager and PCN Leads to identify specific groups for targeted interventions Support with development and delivery of improvement plans to increase cancer screening rates Work within the community to raise awareness and confidence in cancer screening across the PCN area Where possible, identify and support individuals who may benefit from additional support to access screeningData, Monitoring
Attend all training on relevant IT and record keeping systems, keeping up to date with any changes Ensure all work and data is captured on relevant Primary Care System as required Capture, record, and evaluate feedback from work with patients to offer ever-improving service Provide data and contribute to reports, articles, statistical records, and other monitoring as requested Provide regular updates on progress and challenges of all aspects of the project to Self Help UK Managers as required Understand data requirements of the project, including evaluation and outcome measuresPolicies and Information

Technical and Occupational Skills
Essential
Full driving licence and access to a car for work use Good planning, organisational, and time management skills. Ability to work as part of a multi-disciplinary team as well as on own initiative, balancing autonomous and collaborative working within the same role Ability to deliver on agreed objectives within deadlines. Strong interpersonal skills tact, diplomacy, persuasion and negotiating skills Ability to communicate effectively in writing and to produce documents in a range of formats to suit a range of audiences Ability to liaise with a wide range of organisations and people, communicating at different levels as required Ability to effectively cope with conflicting and complex demands Emotional resilience to deal with challenging situations and the hardships faced by patients/carers Ability to handle confidential and sensitive information Good IT skills and knowledge (Microsoft Word, Excel and Outlook).
