The successful applicants will join the Yorkshire Ambulance Service MERIT (ground based critical care team), working alongside Specialist Paramedics (Critical Care) to deliver care to a variety of critically ill and injured patients across the region, including (but not limited to) major trauma and medical, surgical and paediatric emergencies.

While on duty, the doctor will cover the 'Medical Advisor' role for the organisation.

Main duties of the job

The successful applicant will respond as part of a team to critically ill and injured patients across the region and will be able to deliver a wide range of pre hospital care including critical care interventions, for example Pre Hospital Emergency Anaesthesia, Resuscitative Thoracotomy, Resuscitative Hysterotomy, sedation and amputations.

The individual will be able to work well as part of a team as well as independently if required, and will be supportive to colleagues giving advice, feedback, support and mentorship as required.

The role also incorporates Medical Advisor for Yorkshire Ambulance Service for the duty hours. This includes giving clinical phone advice to Yorkshire Ambulance Colleagues when requested.

In the event of a major incident during MERIT duty hours, the duty MERIT doctor will undertake the role of Medical Advisor, providing advice and support to the Ambulance Tactical Commander.

Job description

Job responsibilities

The doctor will form part of the Yorkshire MERIT Critical Care Team, working alongside a Specialist Paramedic (Critical Care) on a MERIT vehicle from Manor Mill Resource Centre Leeds.

The role will form part of a rota that provides cover from 1000-2000, seven days per week, and during this time will be tasked by the Yorkshire Ambulance Service Critical Care Desk to attend higher acuity incidents where the attendance of the team may be beneficial to patients.

The team will assess and treat patients as part of a critical care team and alongside colleagues from the Ambulance and other Emergency Services. They will provide care at the scene of an incident and where clinically necessary, en route to hospital with the patient.

The team will be able to deliver critical care interventions including but not limited to Pre-Hospital Emergency Anaesthesia, Resuscitative Thoracotomy, Resuscitative Hysterotomy, Advanced Analgesia and Sedation.

While on duty, the MERIT doctor will also take on the role of Yorkshire Ambulance Service Medical Advisor.

During business as usual, the Medical Advisor will offer clinical advice and support via phone to Yorkshire Ambulance Service crews as required, although this will primarily relate to Trauma and Critical Care enquiries which cannot be resolved by the Clinical Hub, Clinical Duty Manager or the Specialist Paramedic (Critical Care) desk.

In the event of a Major Incident within Yorkshire, the role of the Medical Advisor will be to support the Ambulance Tactical Commander either at the scene of the incident or a remote Tactical Command Group, providing medical advice with overall responsibility for medical resources deployed to the incident, the utilisation of these assets and the provision of appropriate clinical interventions for patients at the incident. The Medical Advisor will also ensure rapid flow of patients from the scene to hospital.

The MERIT doctor will also provide support and mentorship to the SPCC cohort while working together and will facilitate joint care, empowering the SPCCs to undertake procedures within their scope of practice with appropriate supervision and support as necessary.

The MERIT doctor will attend and contribute to eight weekly governance meetings and will adhere to the governance requirements of the team.

There will also be two training days per year which team members will be required to attend.

Applicants will also be able to express an interest in a MERIT "Consultant" post prior to interview. These posts attract an additional 1 PA per week to undertake specific supporting roles, as well as to form part of the MERIT leadership team.

The consultant will form part of the Yorkshire MERIT Critical Care Team Leadership team.

As part of the leadership team, the consultants will work with the AMDCC to provide leadership and guidance to the running and development of the MERIT team.

As well as day to day leadership responsibilities, each of the appointed consultants will have specific responsibilities.

These are:

Blood lead and training lead. Rota coordinator and IT development lead. Ultrasound and new equipment lead.

Blood and Training Lead

This consultant will be the link between the team and the MYTT transfusion department. The consultant will oversee governance for blood transfusions and will work with MYTT to continually review and develop the service.

This consultant will be responsible for the development and delivery of training for the team, including Simulations and Daily Drills, as well as clinical updates.

Rota coordinator and IT Development Lead

This consultant will be responsible for developing and managing the rota, including a move towards a sustainable self-rostering system as well as overseeing leave requests.

The consultant will also lead on the implementation of a new electronic database to assist with clinical governance and audit.

Ultrasound and new equipment lead

This consultant will also be responsible for the introduction of ultrasound to the team including the development of a business case, the development of SOPs and training and then ongoing governance.

The consultant will also take a lead on the introduction of new equipment for the team.

Corporate Responsibilities

It is the responsibility of each member of staff to ensure that they maintain the confidentiality and security of all information in accordance with the requirements of the General Data Protection Regulation (GDPR), Data Protection Act 2018, the common law duty of confidentiality and, for the protection of patient confidentiality, the principles of Caldicott. Staff must be aware of and adhere to the provisions of the Health and Safety at Work Act and to ensure their own safety and the safety of colleagues and patients. Mandatory training requirements that are relevant to the post must be decided during Personal Development Review and a training plan developed. Staff should be aware of their individual responsibilities under the Equal Opportunities Policy and ensure that they adhere to the provisions of the policy. Individuals are also required to comply with the policies, procedures and protocols in place within the Trust. Safeguarding Children

To recognise and report to the appropriate authorities any concerns regarding Child Protection issues to include possible child abuse. Under section 11 of the Children Act 2004 all NHS staff must ensure that their functions are discharged with regard to the need to safeguard and promote the welfare of children (Working Together to Safeguard Children 2015).

Safeguarding Adults

To recognise and report to the appropriate authorities any concerns regarding Vulnerable Adult issues to include social care and mental health. All staff must comply with mandatory training requirements and ensure that adult safeguarding is embedded as an essential part of their daily practice.

The shift pattern will initially be one fixed ten hour shift per fortnight, and one weekend (two shifts) every twelve weeks, although it is hoped to moved to an annualised rota pattern within the secondment.
