Job summary

This is an exciting opportunity for a person who is passionate about supporting patients with rehabilitation at home.

The role is to work with our friendly Discharge to Assess (D2A) team supporting Key workers (KWs) to include Occupational Therapists, Physiotherapists, Reablement Practitioners, Nurses and other team members.

The role is community based, working with patients at home undertaking therapy/rehabilitation programmes to support and encourage them to achieve their goals, as directed by KWs. Aiming for optimum level of independence and quality of life,

Main duties of the job

To work as an integral member of the D2A Service providing effective support to Key workers (KWs) to include Occupational Therapists, Physiotherapists, Reablement Practitioners, Nurses and other team members. To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life, as directed by KWs. To work with patients in a range of community locations, mainly in service users own homes under guidance from KWs. To work alongside the D2A team, relatives, enablers and carers to assist the service user to work on goals and promote independence in their own home. The post holder will be expected to work independently but have support and supervision from senior members of the team. To link with the wider multi-disciplinary team across health and social care to help meet the needs of the service user.

Maths & English GCSE or equivalent, NVQ Level 3 (or agreeable to work towards) and/or relevant experience Experience of rehabilitation/reablement Evidence of good communication skills Evidence of being able to work independently and part of a team

Experience of working with frail and elderly
