Job summary

Do you have a desire to work in a community mental health setting, but perhaps feel you would need some development experience to feel confident in this setting? If this is you then this might be the post for you. We are offering a successful band 5 a development post in which you will be supported through a competency framework to gain a band 6 position. This will be a structured development programme that will be tailored to meet the needs of the successful applicant.

We are seeking a Mental Health Practitioner (Mental Health Nurse, Occupational Therapist, or Social Worker) to join our Community Mental Health Team which covers the population of East Southampton. The successful applicant will provide recovery focused interventions which support service users to gain the necessary skills and support to begin to manage their mental health needs.

The work is varied and challenging, and there is a strong team approach ensuring good support for the successful applicant through MDT working and supervision. You will need to have a flexible, multi-disciplinary approach, have a good understanding of risk assessment, crisis planning and the recovery model.

Main duties of the job

*To act as a named practitioner or care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

*To undertake ongoing assessment, treatment and evaluation for service users known to the service.

*To work in a flexible hours service, including providing services outside of office hours and at weekends and bank holidays.

*To co-ordinate the care of an agreed caseload of service users with severe and enduring mental illness.

*To formulate care plans in line with identified need.

*To provide recovery focused therapeutic interventions in line with the agreed care plan and risk assessment.

*To assess risk, develop risk management plans, review and adjust in line with the service user needs.

*To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

*To use tools which promote user empowerment

*To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the postholder provides those interventions.

*To have a proactive approach to the role and needs of carers or families.

*To be innovative and responsive to individual needs tailoring care accordingly.

*To be proactive in monitoring mental health needs and respond in a timely fashion if needs change.

*To participate fully in caseload supervision and management, to ensure that care plans are matched to need and service users are able to flow through pathways of care.

*To work in line with all relevant policies and procedures, including care planning, risk and safeguarding.

*To alert the Team Leader or Manager to any concerns in a timely manner.
