Job summary

Gateshead Inner West PCN are seeking a caring, compassionate and committed nurse who is self-motivated, hard-working, and patient focused to join our team.

The post holder will work closely with our GP's and other primary care professionals within the PCN to proactively identify and manage a caseload of patients within the community, making sure that support and care is made available to them and that their complex needs are addressed.

This role is offered on a full-time basis, for hours per week, Monday - Friday.

Main duties of the job

To support the organisation and facilitation of clinical care, in a holistic and proactive manner, to patients registered within our PCN. To maintain a high professional standard of nursing care, in adherence to all PCN protocols, policies and guidelines, as well as to provide nursing assistance to the GP's and other members of the primary healthcare team To demonstrate flexibility, in line with the present atmosphere for change within the health service delivery. To work within your own level of competency, and expanding the role in accordance with PCN requirements and own scope of practice

Based in the Inner West of Gateshead our Primary Care Network serves over 45,000 patients across our five member Practices of: Whickham Medical Cottage, Chainbridge Medical Partnership, Glenpark Medical Practice, Teams Medical Practice & Sunniside Surgery.

We provide integrated services to our local population. Aiming to deliver more personalised, coordinated health and social care for people close to home.

The successful candidate will work within the community of our locality across all five sites.

Clinical Duties

Delivering highquality, compassionate care for older & vulnerable people living at homeeither alone or with their family so that they receive proactive care thatsteers them away from avoidable unscheduled care. Toprovide a continuously improving service in line with the clinical governanceframework of The Primary Care Network Practices as well as the local andnational drivers of older peoples services. Thepost holder will work as a member of the PCN Nursing team but will need to beflexible to the needs of given practice within our network. Undertakeclinical tasks when required. Playan integral role in the delivery of vaccination campaigns. To assess, plan,implement and evaluate care plans for patients; promoting proactiveindependence and autonomy; working within a multi-disciplinary team. To work in collaboration with otherprofessionals to deliver and ensure care is delivered appropriately. To demonstrate clinical effectiveness ofservice delivery by use of evidence-based practice and outcome measures. To have the skills to take a history,assess, examine, diagnose, and develop a management plan. To recognise, assess and manage riskacross the immediate and wider working environment and make appropriatedecisions autonomously ensuring statutory requirements are met. To be responsible for patient safetythrough knowledge of systems, legal requirements and understanding oflitigation. To communicate effectively verbally andin written form in the exchange of highly complex, sensitive, or contentiousinformation in demanding situations using de-escalation, mediation, resolution,and professional Duty of Candour. To evaluate care, taking appropriateaction leading to improvement in quality standards through clinical audit, rootcause analysis and dealing with complaints.

Demonstrable nursing experience across a variety of settings Experience of delivering patient education and promoting self directed care Experience of working with patients with long term conditions Experience of working within a nursing team Experience of identifying, assessing and implementing care for frail patients Registered as a General Nurse with NMC Nursing Experience within General Practice and/or Primary Care Excellent communication skills (written and oral) Strong IT skills Clear, polite manner Competent in the use of Office, Outlook & EMIS Effective time management (planning & organising) Ability to work as a team member and autonomously. Excellent interpersonal skills
