Main duties of the job

This is a rewarding role, you will have the time to provide care planning, admissions avoidance, home visiting, and support with the management of an ongoing caseload. There is a weekly MDT with support from a UHD Geriatrician, enhancing the management of these complex patients. This is an autonomous role with the opportunity to work collaboratively with the hub team and shape what is delivered and the structure of how this support is provided across the area.

We are a forward thinking, friendly, federation of practices looking for a GP who has an interest in elderly medicine to help support a community team of healthcare professionals caring for patients in Poole North and East Dorset.

Although we have an office at 9 Mitchell Road, this role will be based partly in the community, partly at the St Leonards hub, partly in practices, and some remote working may be available depending on the needs of the patient and business.

Qualifications

Essential

You will be a fully qualified and practicing GP and this role would suit someone who either wishes to make this their only employment, or someone who wishes a portfolio career, taking on this role in addition to their other work. We are happy to discussion sessions and flexibility.
