Job overview

The post has been created to cover a 12 months fixed term position in the service.

The post holder will be required work as part of the multidisciplinary community PLD health team in Brent. The team provide a specialist service to adults with learning disabilities in a range of community environments across the borough of Brent.

The post holder, as part of the Community Learning Disability Health Team is responsible for providing high standards of clinical assessment and intervention relevant to their specialist area. S/he will work with a range of other professionals, including physiotherapists, occupational therapists, psychologists, psychiatrists, social workers and a range of other carers and community staff. S/he may encounter a wide range of physically and emotionally challenging conditions in the clients’ homes and other environments in the community.

The post will involve working in a range of community environments including client’s homes in Brent.

To work as an integral member of the learning disabilities team enabling people to access where appropriate mainstream health services.

To manage a caseload of clients with learning disabilities who have a range of health needs, using evidence-based/person-centred principles to plan, implement and evaluate interventions within the Community Learning Disability Team.

Main duties of the job

• To maintain an active caseload of service users within a specialised area, 
including challenging behaviour, mental health and or autism
• To give nursing support and advice in relation to health care to people with learning disabilities and their families/carers.
• To participate in the assessment, planning, implementation and evaluation of nursing interventions.
• To undertake, record and review risk assessments pertaining to the service user and the environment.
• In conjunction with the senior community nurse, prioritise workload in order to deliver a fair and equal service based on clinical need.
• To work autonomously and organise workload – referring to the senior community nurse where appropriate.
• To utilise and participate in regular clinical and peer supervision using reflective practice in accordance with clinical governance.
• To develop and maintain a positive therapeutic relationship with service users and carers.
• At all times adhere to the Nursing and Midwifery Council code of Conduct.
• To maintain records of all visits, communications and other activities concerned with the care of individual clients.
• To participate in the MDT rota chairing and note taking
• To promote best practices in health and safety.
• To ensure clear, factual and accurate information is meaningful and accessible to people with learning disabilities .
• To assist the senior community nurse with mentoring and assessing student placements.
