Job summary
SWAGGA PCN are seeking an enthusiastic, patient-focused, andindependently motivated person to join our existing pharmacy team to providequality patient-care. We want a person with energy and interest to develop thePharmacy Technician role through collaborative working.
This is a new and developing role taking advantage of the NHSNetwork Contract DES for the Additional Roles Reimbursement Scheme.
The successful candidate will work across the 16 GP Practices inthe PCN, with projects ranging from dealing with prescription requests,cost-saving switches to facilitating medication reviews and systems and processimprovements. We expect contribution to the overall medicines optimisationprogramme acting on best practice models from local and national guidelines.
This role will be critical to realising our ambitions to improvepatient outcomes in practices, care homes and domiciliary settings.

Main duties of the job
1. Lead improvements to maximise safe, cost effective and bestpractice in prescribing to improve the quality of patient care.
2. Be a skilled professional to help patients to get the bestfrom their medicines by switching medications to agreed and approved protocols.
3. Improve repeat prescribing processes in General Practice,including promotion of repeat dispensing and online ordering.
4. Minimise clinical risk and aiming to reduce wasted medicines.
5. Support patients on discharge from hospital, requiringexcellent communication skills.
6. Being a Pharmacy Technician in the PCN will be an evolvingjob role with huge scope for development as we expand our PCN team further overthe years.

Essential
Experience of participation in audits or project management Experience working with GP Practices and the law related to medicines use An appreciation of drug actions, doses, formulations and interactions. for commonly prescribed medications. Familiarity with NICE technology appraisals relevant to primary care. Experience of EMIS or previous experience within a comparable role in general practice. Good communication skills both written and verbal. Ability to recognise and work within bounds of own competence. Good IT skills. Completed the CPPE Pathway.

The candidate must be a Pharmacy Technician registered with the General Pharmaceutical Council (GPhC). Academic/vocational qualifications BTEC in Pharmaceutical Sciences or equivalent and have a NVQ level. Significant relevant and ongoing CPD Be enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)-
