Job summary

We are looking for one Band 7 Diabetes Specialist Nurse to join our team at Imperial College Healthcare NHS Trust (ICHT) which is comprised of five acute hospitals. Working as part of a multidisciplinary team with consultants, podiatrists, dieticians and other professions, the diabetes nursing team perform a crucial role in making a difference to patients with diabetes, frontline staff and a service provider in Northwest London.

The diabetes service at ICHT is a comprehensive acute service offering outpatient clinics specialising in:

Ante-natal care

Enhanced type 1 service for insulin pump therapy and continuous glucose monitoring

Structured education for type 1 diabetes

Transition clinic for young adults

Specialist foot unit

Joint clinics for complex type 2 diabetes and obesity, liver and HIV

Non-classical diabetes

Diabetes and renal clinics

Nurse-led clinics for young adults, type 1 diabetes and complex type 2 diabetes

Post-holders will also need to support patients with diabetes on inpatient wards , Elective and non-elective admissions based on an e-referral system

Main duties of the job

Provide specialist diabetes nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets and in accordance to NHS standards and guidelines.

The post holder will provide specialist diabetes nursing care and support and deputise for their line manager in the management of their clinical speciality. This will involve dealing with complex situations and having to take appropriate action in a variety of inpatient and outpatient settings across 3 sites when required.

To be flexible in the approach to the diabetes service and to provide cover for any area specified acting as professional role model providing direct and indirect clinical care, teaching and research.

Education/ qualifications

Essential

Registered Nurse Post registration training in diabetes at level 6/7 Teaching and Assessing qualification or equivalent teaching experience Significant experience at Band 6 / Band 7 as a Diabetes Specialist Nurse DAFNE trained

Desirable

Degree in Health Related subject and Masters Non-Medical Prescriber or willingness towards it

Experience

Essential

Significant post registration experience in clinical diabetes Significant experience managing patients with T1DM and T2DM Proven teaching and assessing skills for both patients, carers and healthcare staff Experience in audit/research/project management

Desirable

Development of local guidelines and SOPS Experience of diabetes technology such as insulin pumps and related software

Skills/knowledge/ abilities

Essential

Completion of TREND competencies or equivalent Knowledge of research and audit including implementation of findings and review of outcomes Good IT skills, Able to manage difficult situations effectively
