Job summary

We are seeking an experienced registered nurse with an interest in respiratory disease management . To be responsible for the delivery of practice nursing services in a friendly, supportive and progressive environment. To work as part of a dynamic multidisciplinaryteam, delivering a high standard of care within their scope of practice to thepractice population.

A Welsh speaker is desirable for this role, but not essential.

Main duties of the job

The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the GP Partners and Practice Manager as appropriate, having regard to current training & regulations. Please see the Job description & person specification for further information.

The Practice area of Meddygfa Taf is predominantly rural, a large farming community, Welshspeaking with a practice population of 7700.

Meddygfa Taf has been a training practice for GP Registrars and medical students for over 30 years. We have a strong training and educational ethos; we understand that we learn by example and sharing of good practice. Our clinical team consists of 6 GP Partners, 2 GP Registrars, 3 Practice Nurses, 2 Health Care Assistants, Phlebotomist, Pharmacy Technician & Cluster Pharmacist. We also work very closely with several external healthcare professionals, such as: Physiotherapists, Occupational therapists, Specialist Nurses, a Social Presciber.

Professional:

Managementof the practice Respiratory Disease Patient Register

Assess,plan, develop, implement and evaluate programmes to promote health andwell-being, and prevent adverse effects on health and well-being

Implementand evaluate individual treatment plans for patients with a known long-termcondition

Identify,and manage as appropriate, treatment plans for patients at risk of developing along-term condition

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare

Supportpatients to adopt health promotion strategies that encourage patients to livehealthily, and apply principles of self-care

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimes, side effects and interactions

Supportand advise women requesting information relating to family planning needs

Supportand manage health needs of women presenting for cervical cytology consultations

Recognise,assess and refer patients presenting with mental health needs in accordancewith National/ Local guidelines for Mental Health

Advise,support and, where appropriate, administer vaccinations for patients travellingabroad

Promoteand deliver evidence-based care for patients presenting with aural conditions

Offeradvice about childhood and adult vaccinations and ensure vaccines areadministered under patient group directions

Assessingproblems presented opportunistically by patients

Able todocument consultations according to NMC guidelines

Ensuresawareness of statutory and local clinical protection procedures, includingsystems of referral. Ability torecognize signs and symptoms of abuse

Providea holistic approach to wound management and implement wound care in line withcurrent evidence-based guidelines

Able to recognizeand manage anaphylaxis according to current UK guidelines

Able toperform Cardio-pulmonary resusitation according to current UK guidelines

Assistin the provision of minor surgery

Abilityto obtain and document informed consent (either verbal or written)

Ensureinfection control guidelines are maintained

Abilityto monitor and manage maintenance of stock and equipment

Confidentialityof information gained at work must be preserved at all times

Managerial:

Contributeto the assessment of service needs

Assistpatients to identify their health needs

Responsible for thecorrect and safe management of the specimens process including collection,labelling, handling, use of correct and clean containers, storage and transportarrangements

Awareness and use of and promotion ofPersonal Protective Equipment (PPE) for the practice including provision,ordering, availability and ongoing correct usage by staff

Promote hand hygieneacross the practice

Identifying the risksinvolved in work activities and undertaking such activities in a way thatmanages those risks across clinical and patient process

Knowledge of practice facilities andequipment in relation to infection control, ensuring that provision of handcleansing facilities, wipes etc are sufficient to ensure a good clinicalworking environment

Safe management of sharps proceduresincluding training, use, storage and disposal

Using appropriateinfection control procedures, maintaining work areas in a tidy, clean andsterile, and safe way, free from hazards. Initiation of remedial / correctiveaction where needed

Actively identifying,reporting, and correction of health and safety hazards

Registered Nurse Post graduate diploma or degree (Primary Care)

Diploma in Chronic Disease Management - Respiratory
