As part of this role you will beaccountable for overseeing and providing direct nursing care toPACE (Program of Allinclusive Care for the Elderly) participants invarious settings such as the clinic care center home virtually andSNF. This position is integral to the Interdisciplinary Team (IDT)and involves collaborating to carry out assessments and driveparticipant care plans. It presents an opportunity to be part of anew culture that emphasizes quality outcomes teamwork and empathyin tailoring care to meet each participants uniqueneeds. KeyResponsibilities:
Play a crucial role in theInterdisciplinary Team (IDT) contributing to comprehensive careplans patient assessments delegation of duties and prompt servicedetermination requests
Lead personalized care for agroup of participants in collaboration with a medical providerbeing the central point of contact for the IDT to ensure progresson a participants care plan
Deliver care alongside teammembers at various locations where the participant is located (homecare center virtual ALF SNF) in areas such as basic primary caremedication administration wound care and other clinical needswithin the scope of practice
Perform nursing assessmentscovering physical psychosocial and behavioral status
Identify emergency situations and independently initiateappropriate therapies within the scope of practice
Educate participants caregivers and staff on customizing andexecuting care plans and responding to participantspecificsituations
Monitor and assess therapeutic interventionsand recommend proactive changes to enhance health andwellbeing Scheduleand Shift Details:
Fulltime position requiring 40 hoursper week MondayFriday with occasional oncall coverage. Schedule mayvary. Benefits:
