Job summary

An exciting opportunity has arisen for a PCN ClinicalPharmacist (IP) to join our ever-expanding team within Tadcaster and SelbyRural PCN working within the Practices.

Tadcaster and Selby Rural (TaSR) PCN are a well established3 Practice PCN with approximately 32,000 patients, covering the largegeographical area surrounding Tadcaster, Sherburn-in-Elmet and South Milford.

The right candidate should be caring, dedicated andcompassionate. They will also need anopen and flexible attitude towards working with a variety of different patientneeds. Good written and verbal communication skills as well as strong time andcase management abilities are also needed.

The post holder will be supported by Clinical Pharmacistsworking for the other Practices within the PCN.

The successful candidate should have a passion and drivefor providing high quality and personalised care. General Practice experience is desirable butnot essential. Prescribing qualificationis essential. The post holder will be required to enrol on the General PracticePharmacist Training Pathway from CPPE if not already completed.

Main duties of the job

The main duties of the role will be to support the PCNPractices with all aspects of medicines optimisation and structured medicationreviews. The post holder will workwithin a multidisciplinary team to provide expertise in the medicine's aspectsof the role. They will work directlyalongside GP's, Advanced Clinical Practitioners as well as other AHP's employedacross the PCN.

About us

The Clinical Pharmacists contract will be held by South Milford Surgery on behalf ofTadcaster and Selby Rural PCN. Thesuccessful candidate/s may be working in one or all of the PCN Practices (SouthMilford Surgery, Tadcaster Medical Centre and Sherburn Group Practice).

The PCN works over 3 Practice sites, has 1 ClinicalDirector and is managed by the Practice Managers and PCN Manager.

The postholder will participate in any training programmeimplemented by the PCN as part of this employment, to include participation inan annual performance review, ongoing continuous professional development,learning and performance.

The postholder will support the quality, diversity andrights of the patient, careers and colleagues to include acting in a way thatrecognises the importance of people's rights, interpreting them in a way thatis consistent with Practice procedures, policies and legislation. Respecting the privacy, dignity, needs and beliefsof patients carers.

Main roles of the job may include but are not limited to:

Patient facing Clinical Medication Reviews Medicines quality improvement Management of common/minor/self-limiting ailments Telephone medicines support Management of medicines in a care home setting Management of long-term conditions Reviewing of safer prescribing and prescribing safetyalerts. Contributing to the achievement of QoF/DES/IIF outcomes. Actioning acute prescription requests. Undertaking clinical audits and reporting findings back tothe wider Practice and PCN team.

The postholder should have an understanding of the widerhealth, social, economic and environmental factors impacting patients. An appreciation of the nature of Primary Careprescribing, concepts of safe prescribing and strategies to improveprescribing.

Qualifications

Essential

Completion of an undergraduate degree in Pharmacist and registration with the General Pharmaceutical Council therefore demonstrating an understanding of and conforming to the relevant stands of Practice Additional Independent Prescribing qualification

Desirable

Successful completion of the Postgraduate Diploma in Clinical Pharmacy Services via the CPPE General Practice Pharmacist Training Pathway.
