Job summary

We are looking for nutritionists to deliver care topatients across our Type 2 Diabetes Path to Remission programme (formerly knownas Low Calorie diet programme) for people with Type 2 diabetes, commissioned byNHS England.

You will provide support and guidance to clients onthe T2DR programme on a face-to-face basis in one-to-one clinics over differentphases of the programme. Clients will sometimes require ad hoc support on thephone or via text messages. Key to thisis motivational interviewing and behavioural change skills, working through anypotential barriers to adhering to the different phases of the programme and keepingpatients committed to achieving their outcomes.

Main duties of the job

You will have the opportunity to attend monthlyclinical supervision sessions and training sessions in order to upskill anddevelop whilst in the role. You will be supported by a team of specialistclinicians throughout and provided with relevant training to help you succeed.

Job description

Job responsibilities

Your key responsibilities will be:

To work as part of a team to deliver the T2DR programme face-to-face to individuals with type 2 diabetes, carrying out telephone consultations as well as communicating with clients via email and secure messaging service To use behaviour changes skills to support clients throughout each phase of the programme by following the guidelines and training provided Taking accurate measurements for instance weight, blood pressure and blood glucose Respond to clients using a variety of communication methods To liaise with supervising dietitian and/or GP regarding any nutritional or medical query as appropriate General day-to-day admin duties to ensure all data is captured in an accurate and timely manner To attend mandatory training and be an active participant in staff meetings

To work as part of the team and on your own.

Qualifications:

You will be required to be a registered nutritionist or working to be a registered nutritionist

You will be required to complete a diabetes management course of a minimum length of 6 months, funded by ABL Health.

Qualifications

Essential

Nutritionist qualification and registered with the AfN

Desirable

Completed or working towards Cambridge Diabetes Education Programme or other formal diabetes management training.

Experience

Essential

Experience working with people to support behaviour change. Experience working with hard-to-reach groups and tackling health inequalities.

Desirable

Experience working within a multi-disciplinary team.

Skills and Knowledge

Essential

Good written, verbal, and non-verbal communication skills, with the ability to build rapport quickly with people. Organised with good administration, record keeping and data collection skills. To be able to work as part of a team and independently as required.

Desirable

Excellent time management and organisational skills. Pro-active approach and ability to work on own initiative. Willingness to learn and innovate and keep up with CPD as per AfN guidance

Job Specific Requirements

Essential

The ability to travel independently across the organisational footprint. Work flexibly and adapt to suit service need including evening and weekend work as required. Must be willing to undertake an Enhanced DBS check.
