Substance Use Nurse

Purpose of the Post:
The Recovery Solutions Team are looking for a Substance Use Nurse to join their growing service. The post holder will deliver high-quality nursing care to service users with substance use needs. They will work as part of a multi-disciplinary team, delivering person-centred substance use interventions and clinical procedures in line with level of training and competence. They will support/manage a caseload of service users with varying degrees of complexity by undertaking comprehensive assessments of substance use needs and developing thorough clinical care plans.

Duties and Responsibilities:
1. Undertaking physical and mental health assessments for people who access our services when indicated by clinical need, as well as ensuring appropriate sign posting/referrals to primary care or appropriate secondary health care services for identified health issues.
2. Delivering specific clinical procedures and tasks in line with level of training and competence
3. Supporting in the delivery of pharmacological and psychosocial interventions including assessments, care planning and review of service user's treatment
4. Managing/supporting a case load of service users with varying degrees of complexity
5. Conducting assessments of physical dependency and determining safe induction/titration to opiate substitute treatments as required
6. Conducting detox assessments- identifying the need for specialist medical referrals and the suitability for community detoxification. Where appropriate, making referrals for specialist inpatient services and liaising with hospital and inpatient unit staff.
7. Supporting in analysis of incident trends and in service user mortality review processes
8. Completing clinical audits and supporting with medicine optimisation and cost effectiveness
9. Ensuring that there is good communication of treatments offered, treatment progress and outcome of any investigations to other healthcare agencies, partner agencies and GPs
10. Effective and collaborative communication with colleagues and management
11. Retaining all required competencies and revalidation
12. Attending and actively participating in meetings appropriate to the development and maintenance of the role
13. Ensuring that procedures are constantly reviewed to promote best practice and evidence based practice in accordance with UK Guidelines on Clinical Management, NICE guidance and Middlesbrough Council protocols and procedures
14. Ensuring a high standard of case notes and that all paperwork and recording is undertaken in accordance with Middlesbrough Council Policy and Procedure
15. Ensuring Health and safety standards are maintained
16. Maintaining NMC nursing standards and administering medication safely in line with PGD
