An exciting opportunity has arisen for a full time Band 6 Deputy Clinical Nurse Specialist to come and join our foot health team within Diabetes and Endocrine team at Guys and St Thomas ' hospital. 

You will need to be self motivated and innovative with an interest in caring for patients in Diabetic Foot Health.

Be flexible to work in Community and cross site. Able to manage a case load of patient autonomously and report back to senior Podiatrist team.

Main duties of the job

You will be expected to have contact with current out patients who may require input from the foot health team and be trained to carry out nurse led consent for a number of procedures and responsible and accountable for the complex assessment, diagnosis and treatment of new and existing patients with a wide variety of Podiatric complaints.

This includes diabetes, rheumatoid arthritis, musculoskeletal disorders, vascular and neurological disorders, systemic diseases, chronic pathologies, emergency treatments, wound management.

You will be aware and recognise the contribution of our assessment within the whole health care package/profile through liaison and referrals for smoking cessation, counselling, prescription exercise and weight control management.

You will also undertake specialist diabetes and high-risk sessions for the assessment and treatment of patients where duties will include: Undertaking Diabetic foot assessments and diabetic foot management and education Working with challenging groups including those with mental health problems Management of foot ulcers To identify risk factors and to prioritise patients into risk categories To arrange emergency admission into hospital when indicated Providing emergency domiciliary care To achieve Independent prescribing status

Detailed job description and main responsibilities

Communication and teamwork are essential and individuals need the ability and confidence to work alone. There is an expectation that you provide education not only for patients but to participate in organised study days for various teams within the hospital. You will need to be a self motivated and innovative individual with a desire to progress in your career.

To provide care in a variety of settings often involving the ability to work alone and independently, and also with the Podiatry team: Hospital wards Multidisciplinary units Acute satellite units

Be competent in management of a deteriorating patient , venepuncture cannulisation and IV medication administration.

Opportunity will also be given for training for further development in Foot Health and non medical prescribing for future career development.

Education and Qualifications

Essential criteria

Evidence of BCE or Degree in adult nursing Evidence of advanced nursing within clinical practice

Desirable criteria

Advanced assessment course

Knowledge and Experiance

Essential criteria

Mentorship in practice qualification or equivalent Evidence of independent and team working Experience of managing acutely unwell adults

Desirable criteria

Experience in wound management

Skills and Competencies

Essential criteria

Evidence of IV competencies Expert in cannulisation and venepuncture Ability to work within MDT tea Skilled in complex wound managment
