The Team maintains our focus on providing quality Home Treatment both during supported discharge and as an alternative to Hospital admission.

You would be joining a Trust that has a Philosophy of Recovery based practise, and you will have the opportunity to contribute to the working within an established CRHT Team.

We are currently in the process of expanding our services and are developing the services in both the Basingstoke and Winchester localities with separate team bases for each service.

Our aim is to provide the kind of Service that we would expect for ourselves, or for someone that we love and care for. A service that promotes self-direction, independence, and choice, that respects the individual, and provides the treatment and help that is needed as a priority.

Main duties of the job

As a Band 6 practitioner you will be taking a lead role in the team, supervising, and supporting other team members, and will be expected to have the skills and knowledge that will facilitate effective care being delivered to service users in their own homes. Exciting opportunities for professional/personal development and career progression are also on offer through our Academy of Clinical Excellence (ACE) available to all of our registered practitioners.

The post-holder will develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery and co-ordinate and monitor the delivery of person-centred healthcare.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
