Main duties of the job
We are seeking a permanent or bank Healthcare Assistant withan interest in palliative care to work in the Community within ourmulti-disciplinary team who provide physical, psycho-social and spiritual careto patients.
The permanent role can be flexible, with part time hours between 30 - 35 per week available. As a bank Healthcare Assistant, we will call you tooffer occasional shifts, this may be planned or at short notice, for a fullshift or as ad hoc additional hours, so flexibility and commitment isessential.
Hours: Part time, 30 - 35 hours per week

Job responsibilities
JOBSUMMARY
To provide care to patients at the end of life or to assist patients to be discharged orremain at home. To be adaptable and to work flexibly and be able to assess andrespond to situations calmly.
CLINICALRESPONSIBILITIES
To carry out agreedtasks involving direct patient care and activities within patients' homes, (IPUand Community Care Unit as required) working within the level of training andcompetence.
To assist inphysical care involving hygiene, elimination, eating and drinking,mobilisation, rest, sleep and comfort needs.
To assist inmeeting the patients and his/her familys spiritual and psychological needs,recognising and respecting religious and cultural beliefs.
To respectthe patients need for social interaction or solitude, recognising theimportance of work and recreation.
To assist inproviding support and comfort for the dying patient and his/her family.
To assist ineffective verbal and written communication between patients, families andprofessionals working in the community and Hospice.
To maintain accurate records ofpatient care.
Toadminister certain prescribed oral medication where the patient is unable toself-administer and in the absence of a capable non-employed carer andcomplying with policies, if appropriate and only after passing competencytraining.
To performcertain clinical tasks, if appropriate and after passing competencies andtraining.
To refer to additional Hospice services or/and District Nurse orFamily Support Worker any symptoms, concerns for either the patient or familyand changes or deterioration of the patient's condition.
To have anunderstanding of safeguarding principles and be able to apply these within everydaypractice and intervention/within your role.

Toparticipate in the development of clinical standards within the total qualityframework of the Hospice and participate in clinical audit, if required.

To respectat all times the confidentiality of information covering patients, staff andvolunteers.
To promotethe Hospice philosophy of care towards patients, relatives and visitors.

Essential
Ability to assess patient needs, training will be given. Demonstrable experience of team working. To have an understanding of safeguarding principles and be able to apply these within everyday practice and intervention/within your role.

Experience of working within a multi-disciplinary team. Community/palliative care experience. Ability to plan, deliver and evaluation appropriate care packages.
