Job description

Job responsibilities

You will work as part of amulti-disciplinary team in a patient facing role to clinically assess and treatpatients using their expert knowledge of medicines for specific disease areas.

You will take responsibility forthe care management of patients with chronic diseases and undertake clinicalmedication reviews to proactively manage people with complex polypharmacy,especially the elderly, people in care homes, those with multipleco-morbidities (in particular frailty).

You willprovide specialist expertise in the use of medicines whilst helping to addressboth the public health and social care needs of patients at the PCNspractice(s) and to help in tackling inequalities.

Provide leadership on personcentred medicines optimisation (including ensuring prescribers in the practiceconserve antibiotics in line with local antimicrobial stewardship guidance) andquality improvement, whilst contributing to the quality and outcomes frameworkand enhanced services.

Through structured medicationreviews, clinical pharmacists will support patients to take their medicationsto get the best from them, reduce waste and promote self-care.

Developing relationships andworking closely with other pharmacy professionals across PCNs and the widerhealth and social care system, you will take a central role in the clinicalaspects of shared care protocols, liaison with specialist pharmacists(including mental health and reduction of inappropriate antipsychotic use inpeople with learning difficulties), liaison with community pharmacists,secondary care and care homes.

The Benefits:

You will have training, supportand continuing personal development facilitated by one Band 8 ClinicalPharmacist employed by a PCN member practice.

You will be required to undertakethe CPPE PCPEP (Primary Care Pharmacist Education Pathway), if not alreadycompleted

You will be required to undertakethe independent prescribing course to become a non-medical prescriber, if notheld.

Role requirements:

Interested? To be considered, youwill need:

Registration with General Pharmaceutical Council

A Masters degree in pharmacy (MPharm), orequivalent

A minimum of 2 years postqualification experience

Whilst not essential, knowledgeof EMIS, Word and Excel would be an advantage as would experience of workingwithin Primary Care.

This role involves an element oftravel between PCN sites as and when required.
