Main duties of the job
Through the four pillars of practice (clinical practice, leadership & management, education, research) you will lead the development and implementation of innovative services within general practice and primary care's interface with other parts of the health and social care system.
As a senior leader you will deliver on national and health board strategies, professionally integrating pharmacy into frontline services as a driver for system change. As a senior practitioner you will deliver frontline patient care, leading by example on the delivery of prudent and value-based healthcare.
Key duties include;
managing patient caseloads designing and evaluating new professionally integrated services collaborating with multidisciplinary teams to integrate pharmaceutical care for optimising medication management and better patient experience and outcomes.You will lead on training and education initiatives for our current and future workforce. You will develop or adopt best practices by contributing to research and quality improvement, raising the expectation of what can be done to shift care from hospitals to the community, and from community to people's homes.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 12,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
47 general practices (6 of which are managed practices), 45 dental practices (including 4 orthodontic), 96 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Master's degree in Pharmacy Registered pharmacist with GPhC Post-graduate diploma/MSc in clinical pharmacy or equivalent experience Independent Prescriber Qualification RPS (Royal Pharmaceutical Society) credentialed Consultant Pharmacist (or working towards this).
