Job summary

An exciting opportunity is available for an experienced Mental Health Practitioner to join our MSNAP Accredited Memory Assessment Service.

The Memory Assessment Service East (CMAS) is nurse-led with a strong emphasis on multi-disciplinary team working to ensure that users of the service receive high quality, compassionate care throughout their memory assessment process. The team is comprised of a Psychiatric Consultant, Clinical Psychologist, Clinical Associate Psychologist, Occupational Therapists, Non-medical prescribers, nurses and clinical support workers, and excellent admin support.

The service is proud of being dynamic, responsive, and proactive to the changing needs of the local population and its service users. We work closely with other multi- agencies services such as Dementia Connect.

The team is currently embracing the upcoming changes of rebranding of the service to Suffolk Dementia Assessment Service East thus supporting the vision of aligning and accredited both East and West Memory Services to the same standard of quality of care across Suffolk. This is a fantastic time to be part of the development of this ambitious plan.

Main duties of the job

The successful candidate will join a service with a positive team culture, focused on learning, developing, improving and supporting each other as well as our service users. Staff wellbeing and development are key priority areas.

General responsibilities include:

- Completing holistic assessments that include the application of a Cognitive Testing tool (training provided). These are undertaken at patient facing clinics or home visits.

- Undertaking Duty/Triage role on a rota basis

- Attending a weekly Diagnostic Formulation MDT meeting to review the outcomes of assessments completed

- Will be delivering diagnoses to service users (Training provided)

- Participating in regular team teaching/ development opportunities thus supporting sharing of best practices.

- Supporting students on placement with the service as either practice assessor and/or supervisor.

- Supporting the facilitation of post-diagnostic groups

- Monthly management and clinical supervision is provided.

- Participation in service development

- Participation in research projects

- Development of robust relationships with partner agencies and services.

-Maintain professional registration and completing of CPD as necessary.

- The role allows for hybrid working where you will be expected to be able to work autonomously and manage your time well.

-You must be able to be able to travel to various locations across Suffolk.
