At Wokingham Community Hospital we have two wards within ourInpatient Unit, caring for adults of any age with a wide mix of health careneeds. We are looking for an experienced Occupational Therapist to join ourteam and provide rehabilitation and discharge planning for our patients, whosneeds range from post-fall recovery to stroke rehabilitation to advancingdementia and everything in between.

We are looking for an enthusiastic and dedicated member ofstaff with effective communication skills, a flexible approach and an abilityto work under pressure, both as part of a Multi-Disciplinary Team andindependently.

This role would be working full time (part time considered) aspart of the therapy team and the wider MDT, both on and off site, liaising withward and Social Services colleagues regularly and effectively.

Main duties of the job

To be responsible for a highly complexcaseload of patients to help improve their confidence in activities of dailyliving and enable the patients to return to their own homes if appropriate.

To provide highly skilled and specialistassessments of occupational performance. Plan, implement and evaluateevidence-based client centred intervention, maintaining the associated recordsin compliance with the standards from the Royal College of Occupational Therapyand the Trust.

To contribute to the provision of anefficient, effective and patient focused Occupational Therapy Service atWokingham Hospital.

To identify service developments &needs, to contribute to the planning, development and evaluation of theservice, ad have responsibility for defined projects.

In addition to ensuring you develop the skills needed todeliver a high standard of personal professional care and treatment to thepatients on our wards, BHFT is keen to provide access to any additionaltraining.

Job responsibilities

Clinical

Toprovide Occupational Therapy input into Community Inpatient Wards.

To usehighly developed and advanced specialised skill to prioritise workload,referring to other agencies where necessary.

Tocarry a complex, diverse caseload

To behighly skilled in using and reporting on standardised and non-standardisedOccupational Therapy assessment to diagnose and analyse highly complex patientneeds in the context of activities of daily living.

Toassess the need for and carry out functional assessments within patients homesand within the inpatient unit.

Toassimilate and use information and assessments provided by other healthprofessionals

Tomanage clinical and personal risk within own caseload and others within theservice areas working as a lone practitioner

Todevise Occupational Therapy treatment plan and goals in collaboration with thepatient, relative, carers and the interdisciplinary team.

To useadvanced skills in analysing, selecting and grading activity to achievetherapeutic goals

Toassess and issue equipment as appropriate to meet the patients needs

Toensure that up to date written and electronic records, reports and activitydata are maintained in accordance with professional and local standards

Toparticipate and contribute to the Interdisciplinary Team decision makingprocess regarding patient care, establishing strong communication networks

Tocommunicate effectively, share information appropriately and workcollaboratively with the team, families and other outside agencies

Toensure effective communication with patient, relative and carers regardingtherapy

Tocontribute to the development and evaluation of the team

Tocontinually update clinically and professionally, maintaining awareness ofdevelopments within the Trust and NHS nationally

To takepart in clinical audit or research, and initiate and facilitate projects

Tofollow departmental and Trust policies and procedures as required

Topromote the profession and service appropriately, including in-service trainingfor other professionals

Tocomply with HPC and the Royal College of Occupational Therapists code ofethics and professional conduct, national guidelines and Trust procedures

Todemonstrate the ability to reflect on ethical issues and to provideprofessional guidance to staff as necessary

Torespect the individuality, values, cultural and religious diversity of patients

Tocritically appraise and modify professional practice

To usespecialist knowledge and teaching skills to provide support and guidance tocare staff and Occupational Therapy staff

Toprovide effective guidance, supervision and appraisal for junior members ofstaff

To beresponsible for the supervision, written assessment and training ofOccupational Therapy students on practice placement

Toattend mandatory training as required

Todemonstrate leadership and contribute to the departments clinical governanceand service development plan

To maintain professional standards according to theRoyal College of Occupational Therapists Code of Ethics

LOCATION/MOBILITY

We may require you to work at or from a different workbase or location from time to time, to meet service demands and deliver anoperational service. Given the geographical nature of the Trust, you may berequired to travel between Trust premises as part of your role.
