Job summary

Leeds Student Medical Practice and The Light Surgery Primary CareNetwork (LSMP and TLS PCN) are looking to recruit a PCN Social Prescriber. OurPCN is extremely diverse with patients mainly being university students or youngprofessionals. We want the most resilient people to join our ever-expandingteam.

The Social Prescriber will enhance the patient experience within the PCN by providing a mix of reactive and proactive communication and effective care navigation. This role is crucial in connecting patients with various services, ensuring they receive timely and comprehensive support. The Social Prescriber will address immediate needs, proactively educate patients about available resources, assist in navigating the healthcare system, and foster strong relationships between patients, providers, and community services. By bridging gaps in healthcare delivery, the Social Prescriber will ensure a positive and inclusive experience for all patients within the PCN.

This role is initially a 12-month fixed term contract with potential to extend dependant on the success of the role. We are open to both full-time and part-time hours for this role.

Main duties of the job

Thesuccessful applicant will be required to conduct both face to face andtelephone appointments for patients referred into the Social Prescribingservice. You will be responsible for a caseload of patients across bothpractices in the network, conducting initial assessment, as well as follow-upappointments as part of an extended will be required towork on-site at both Leeds Student medical Practice and The LightSurgery.

Thesuccessful applicant will be required to work on their own initiative as wellas being a good team player. You will need to be a good listener, empathetic,and have an excellent understanding of patient needs. You will also haveoutstanding interpersonal skills and be able to work with patients to identifywider health, wellbeing, and social interventions.

LSMP and The Light PCN is 1 of 19 Primary Care Networksacross Leeds, serving over 63,000 patients through our two practices: LeedsStudent Medical Practice and The Light Surgery.

Our multidisciplinary team is composed of highly trainedprofessionals, including clinical pharmacists, pharmacy technicians,occupational therapists, care coordinators, mental health advisors, health andwellbeing coaches, social prescribers, mental health nurse specialists, andcommunity wellbeing connectors. We aim to enhance general practice support byoffering a tailored range of services to meet the unique needs of our patientpopulation and improve access to appointments.

Job responsibilities

The overall aim of this role is to deliver individualised, tailored support to improve thewellbeing of our registered patients. This will be via holistic assessments andintegrated care plans to improve the health and well-being for patients with a longer-termoutcome of optimising the number of medical interventions required.

The successful candidate will be kind, empathetic andtrustworthy to help put patients at ease. They will be knowledgeable in thefield of personalised holistic care and will ideally understand the optionsavailable within the healthcare system to optimise patient care. They will bepassionate about reducing health inequalities, and understanding of thechallenges that impact good patient care. They will be motivated to developthemselves and their role to meet the requirements of the PCN.
