Job summary

Cancer Patient Pathway Navigator Band 4 - hours per week - 18 months Fixed Term

The post holder will act as the patient's navigator tracking their timed pathway against the national guidance.

The post holder is responsible for supporting patients and their families through the relevant cancer pathway across secondary, community and primary care as required by the patient. They will be responsible for coordinating care, education and support for people with complex care needs and providing a point of access to patients and carers. This post is part of a new patient pathway that aims to deliver a diagnosis within 28 days from referral.

The development of the role will be led by the needs of the patients. The post holder must be adaptable and willing to work flexibly as the service progresses.

They will be responsible for entering accurate data onto the Trust's Electronic Patient Record and Somerset Cancer Register database

Main duties of the job

- Coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent needs - Provide advice and refer on or sign-post to other sources of support- Coordinate diagnostic and monitoring through appointments and investigations- Coordinate face to face appointments for those with complex results.- Discuss with patients issues relevant to their health and social care and provide support where possible or escalating relevant clinical teams if necessary.- Maintain accurate patient records on both the Trust Electronic Patient Record and the Somerset Cancer Register to reflect the care given.

GCSEs (or equivalent) including English and Maths at grade C or above. ECDL or equivalent demonstrable skills NVQ Level 3, BTEC or equivalent in a relevant field

Competent in the use of Microsoft Office software Demonstrate ability to use an NHS Patient Administration System Ability to communicate effectively with colleagues, clinicians and managers both internal and external to the Trust Ability to work on own initiative and as an effective and responsible team member Ability to work to agreed time scales in line with policy and guidance by organising workload to meet priorities Ability to co-ordinate others in complex situations Skills in data management and analysis, with a focus on accuracy and attention to detail.
