Job summary

We arelooking for highly motivated Physiotherapist to join our locality-based communitypalliative care teams. Farleigh Hospice is a high performing organisationproviding specialist palliative care and end of life care for people with lifelimiting illness in the Mid Essex region. These locality teams aremultidisciplinary, consisting of a range of disciplines including nursing,therapy, medical, psychosocial and care staff.

Main duties of the job

Based in aspecific locality of Mid Essex, the extensive MDT will deliver holistic care topatients at all stages of end of life from stable to unstable, deterioratingand dying in line with the national palliative care classification. Theteam will also provide essential support to carers and family members. Thiscare may be delivered in the patients home, in our Inpatient unit or as anoutpatient individually or in groups.

You will bean empathetic Physiotherapist with a range of experience including holisticassessment and complex symptom management. You will have excellentcommunication skills and a passion for service improvement withinhealthcare. You will build and maintain strong relationships withcolleagues and other health and care professionals to deliver the best possiblecare for our patients ensuring Farleigh Hospice is as an integral part of thelocal health and care economy.

You will besupported by a dedicated team and have access to excellent professionalleadership and clinical supervision.

Job responsibilities

As a Clinical Specialist Physiotherapist,you will work within a multi-disciplinary team providing specialist advice and care to hospice patients and their families.

Through holistic assessment,provide information, advice,psychosocial and spiritual care to patients with palliative and end of life care needs, supporting their families and carers, liaising and referring on as necessary to appropriate health and social care professionals.

Working partnership with those receiving care, helping them to access relevant health and social care, information and support when they need it.Support people to access, identify, appraise and interpret complex information, to enable them to determine their own future.

Use advanced communication skills in delivering highly emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means.

Work collaboratively as part of the multi-disciplinary team (MDT) sharing the clinical caseload of patients/carers within the locality for patients who have complex palliative care needs to ensure efficient and timely care is delivered.
