Job summary

The Care Coordinator is a non-clinical and diverse role in the Primary Care Network (PCN).

This is an exciting opportunity to obtain an overview of all PCN services and liaise with a cross section of stakeholders from PCN staff, practices, patients and other healthcare and community providers, acting as a central point of contact.

Main duties of the job

Provide a first point of contact for patients and clinicians in coordinating patients care. Encourage people, their families and carers to provide feedback and engage fully in the care coordination process. Ensure timely follow up and action for patients from communications from community and secondary care. Monitor tasks to ensure they are completed and care delivered through regular audit of the clinical system. Signpost and organise appointments, follow ups or other actions to help the PCN provide high quality, compassionate care to our patient population. Work sensitively with patients, their families and carers to capture key information, enabling comprehensive and accurate records of support. This role will also have the opportunity to contribute to wider PCN projects. The PCN may require other support when needed such as providing cover for colleagues when on leave.

About us

We are a group of 8 GP practices working collaboratively since July 2019.

Our vision is to share best practice and make meaningful changes together so that we can give better care to our patients.

This is an organisation while continuing to provide the usual GP services for our respective patients would work to co-ordinate activities around patients groups requiring continued care.

We hope to work with a wider group of authorised providers where necessary to improve patients care pathways.
