The primary responsibility of the AGE WELL CCPL will be to provide strategic leadership, operational insight and clinical input to our ageing well strategic priority. Main duties of the job

o Facilitating the development of a clinical vision for ageing well in Greenwich, making the Fuller vision a reality in Greenwich, through better joining up care and improve population health outcomes through to end of Providing clinical leadership to redesign and better integrate how on the day, primary and urgent care demand is best managed across the whole system in Greenwich covering the primary care, urgent care centres, NHS 111 services, out of hours GP visiting services and maximising the potential of enhanced access, Assistive Technology Enabled Care, Pharmacy Providing clinical leadership for the development of our Home First model, which includes frailty, working with partners on joining up the approach for enhanced health in care homes, frailty teams, Joint Emergency Team and Virtual Wards.

Job responsibilities

Playing a key role with our emerging primary care collaborative, building future approaches in conjunction with our Primary Care Networks on model development, to build encourage the leadership and sustainability of primary care model of the future Being the Age Well representative within the Healthier Greenwich Partnership and, ensuring there is alignment and appropriate resource allocation to deliver on the Health & Wellbeing Strategy and annual plans Quality is a key enabler - Provide clinical leadership for Quality Improvement work across Greenwich, in conjunction with the Senior Quality Manager. This will involve promoting improvement, innovation, address variation with a focus on patient safety and quality, leading by example to foster a quality improvement culture within primary care.

Qualifications

Essential

Qualified care and/or clinical professional NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.

Desirable

Certificate / qualification in programme management, service improvement or transformation

Experience

Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
