We are lookingfor a motivated RMN to join our team at Syrena House, a 9-bedded, male in-patient recovery unit. If you have a passion for promoting recoveryfocused interventions and supporting people to reach their optimum level offunctioning utilising resources within an in-patient setting whilst linkingwith what the community and our stakeholders this might be the post for you.

Main duties of the job

The Post holder will be responsible for theassessment, planning, implementation and evaluation of programmes of care usingthe Personalised Care Framework and will be required to act as a Keyworker foridentified individual service users. The post holder will be required to takecharge of a shift on a regular basis.

The post holder will have the ability to sustainrespectful, consistent and reliable therapeutic relationships with serviceusers that enhances their ability to build social networks and diminishes theirsocial exclusion, maximise their strengths and interests and increase theirparticipation in meaningful activities.

The focus is to support discharge pathways forservice users and to provide individualised care that is supported byco-produced personalised care. This needs to demonstrate evidence of riskassessment and involvement of the relevant MDT members and carer involvement.

The post holder will be required to create andmaintain a learning environment for the development of junior staff and be ableto undertake the role of practice supervisor/assessor.

The post holder will also be responsible for theirown professional development.

The post holder will work at all times within theguidelines of the NMC Code.

Whilst the post is based at Syrena there may be anexpectation to work within the wider Recovery Service at the request of theManager according to clinical and/or as part of personal development.

Job description

Job responsibilities

Todeliver a high standard of individualised care, which will include theassessment, planning, implementation, review and evaluation of the PersonalisedCare Framework. This should be in partnership with the service user, carers andmulti-agency professionals as appropriate. This requires a complex level ofcommunication in the receiving and transmitting of information to serviceusers, carers, colleagues, the wider service and the general public on anindividual or group basis. This will be over a wide range of complex andsensitive issues and will need to take into account barriers to understanding.

To act asKeyworker for an identified group of clients using the Personalised CareFramework process and keep all associated paperwork up to date in a timelymanner.

To ensurecomprehensive risk assessment, history, planning and management is recorded andcommunicated to the team and the wider service if appropriate, for individualservice users.

Tocontribute to and be an effective member of the multi-disciplinary team. Thiswill include leading handovers and MDT meetings. Involvement in the assessmentof new referrals and to attend and contribute to reviews, clinical meetings andcase conferences. It is the role of the nurse to convene necessary meetings,invite the relevant stakeholders, communicate and record, action andcommunicate outcomes.

The postholder will act as a role model to service users and staff. There is anexpectation that they will promote a recovery ethos to encourage service usersin all aspects of activities of daily living. This will include providingverbal prompting and when required additional support to ensure that all theirbasic physical health and personal care needs are met to a high standard. Theseinterventions are also seen as opportunities for assessment/monitoring and/orlearning opportunities.

The postholder will contribute toward the recovery process and the care pathway byutilising their skills and knowledge in a multi-disciplinary way to provideholistic care. This will be achieved through one-to-one work, group activities,networking and co-facilitation of psychotherapeutic groups. Contributing to thetherapeutic programme within the unit and the creation of individual programmesfor service users is a key responsibility.

To workin partnership with carers of clients using Triangle of Care initiatives,offering support, advice and reassurance on often quite sensitive and emotionalissues and at times of high levels of distress.

To beresponsible and accountable for the safe storage and administration ofmedication as per national and Livewell Southwest policies and procedures. Toeducate clients and carers regarding the potential benefits and side effects ofmedication and feedback information to the medical staff/community pharmacist.

The postholder is required to comply with all infection control policies which includesadherence to bare below the elbows.

Leadership

To takeresponsibility for the Nurse in Charge role of the unit in the absence of moresenior staff, including the planning and organisation of the daily clinicalactivity, covering adjustment of staff rotas during the out of hour period andrecording as necessary, flexible changes to routines in response to individualservice users need and changing staff levels and to maintain the safety ofstaff and clients.

Tocontribute to staff development and demonstrate work activities to new orinexperienced staff contribute to induction of new staff when joining ateam/temporary staff who are working shifts.

Provideadvice for staff and direct to appropriate resources which facilitateevidence-based nursing practice.

To beaware of and appropriately implement the policies and procedures relevant toLivewell southwest and keep up to date with their contents.

To beresponsible in attitude towards economy of resources and care of equipment andproperty including the day-to-day financial management of petty cash; safestorage and recording of patient monies; stock control of all supplies stationary, medicines, food office supplies etc and the health and safety ofthe environment.

To beresponsible for the safe use of unit vehicles and the accurate recording ofmileage. To be responsible for carrying out risk assessments and planning toreduce risk when transporting patients.

Toundertake appraisal and line management supervision of junior staff asallocated or as part of ongoing personal development.

ProfessionalDevelopment and Education

The postholder will undertake a comprehensive induction, mandatory and other essentialtraining as directed by the Livewell Southwest and Unit/Deputy manager. Thiswill include Physical Intervention Training which is essential for this post.

Competencieswill be identified through personal development needs and appraisal.

The postholder will be responsible for their own practice, managerial development andto keep up to date with evidence-based developments in mental health nursing.This will include post registration education and preparation of portfolios forrevalidation.

The postholders profession will be maintained and support for opportunities and theways that their skills and knowledge can be used to meet requirements forrevalidation will be central to the line management and appraisal process.

The postholder will be required to ensure they receive line management support andparticipate in clinical supervision and reflective practice.

ClinicalGovernance

Tounderstand responsibilities under clinical governance to comply with EssentialStandards of Quality and Safety as regulated by the Care Quality Commission.

Tocontribute to any audit process within the Livewell Southwest as appropriate.
