Weare looking for an enthusiastic, dynamic GP to add to ourbrilliant and diverse primary care team of social prescribers, pharmacists, MSKPractitioners, MH Practitioners, nurses and healthcare assistants, advancednurse practitioners, physician associates, care coordinators, salaried GPs andGP partners.

Main duties of the job

Yourmain duties may include:

In accordance with the Organisations timetable, thepost-holder will make themselves available to undertake a variety of duties,including face to face consultations, telephone consultations and queries,visiting patients at home, checking and signing repeat prescriptions anddealing with queries, paperwork and correspondence in a timely fashion Making professional, autonomous decisions relating topresenting problems, whether self-referred or referred from other health careworkers within the organisation Assessing the health care needs of patients with undifferentiatedand undiagnosed problems Screening patients for disease risk factors and early signs ofillness Developing care plans for health in consultation with patients andin line with current practice disease management protocols Providing counselling and health education Admitting or discharging patients andreferring to other care providers as appropriate Recording clear and contemporaneous consultation notes to agreedstandards Collecting data for audit purposes Compiling and issuing computer-generated acute and repeatprescriptions (avoiding hand-written prescriptions whenever possible) Prescribing in accordance with the practice prescribing formulary(or generically) whenever this is clinically appropriate The post-holder will undertake all thenormal duties and responsibilities associated with a GP working within primarycare.

Job responsibilities

1. Deliveringa quality service

Recogniseand work within own competence and professional code of conduct, to: Contribute to clinical governance activities / clinical audit toensure continual improvement of service delivery. Contribute to complaints responses / reviews, significant event,peer review and other shared learning activities. Provide services to the practices patient population to thehighest possible standard ie QOF Take action if standards are not met (self or others). Work to agreed standards, monitor and maintain the standard ofcare offered. Effectively manage own time, workload and resources. Utilise the audit cycle as a means of evaluating the quality ofthe work of self and the team, implementing improvements where required In partnership with other clinical teams, collaborate on improvingthe quality of health care responding to local and national policies andinitiatives as appropriate Evaluate patients response to health care provision and theeffectiveness of care Support and participate in shared learning across the practice andwider organisation

2. Communicationand Relationships

Demonstrate sensitive communication styles to ensure patients arefully informed and consent to treatment Act as an advocate when representing patients and colleagues Produce written documents that evidence the contribution of the clinicalteam to the practice priorities Provide and receive confidential and complex information from arange of sources, and escalating as required. To identify and use the most effective means of communication,appropriate to the situation and individuals involved. To manage own behaviour in a positive manner to maintain effectiveworking relationships To always behave professionally and uphold the reputation of the Practice To observe complete confidentiality of all information includingawareness and adherence to the legal requirements of the Data Protection Act.

3. Analysisand Judgement

To ensure you are competent and appropriately trained to carry outyour areas of responsibility. Ensure you undertake mandatory and statutory training requirements Manage and assess risk within the areas of responsibility,ensuring adequate measures are in place to protect staff and patients Monitor work areas and practices to ensure they are safe and freefrom hazards and conform to health, safety and security legislation, policies,procedures and guidelines Apply infection-control measures within the practice according tolocal and national guidelines Advocate for policies that reduce environmental health risks, areculturally sensitive and increase access to health care for all Interpret national strategies and policies into localimplementation strategies that are aligned to the values and culture of generalpractice Recognise and inform managers of any matters requiring theirimmediate attention or expedite and progress issues with appropriate lead, asand when necessary, to ensure that deadlines are met. Participate in the local implementation strategies that arealigned to the values and culture of general practice

4. Planningand Organising

Prioritise own and others workload and ensure effectivetime-management strategies are embedded within the culture of the team Expectation to be flexible in undertaking any other duties asdeemed appropriate to the role

5. Serviceand Policy Development

To be aware of relevant Practice related policies/proceduresrelevant to the post. Develop clinical standard operating procedures with leads, whererequired.

6. PersonalDevelopment

Take personal responsibility to ensure any mandatory training isundertaken. Be willing to undertake additional training and development asidentified at annual appraisal. Works in conjunction with senior management, assessing ownlearning needs and undertaking learning as appropriate.

7. InformationSystem use and management

Use relevant IT systems to support role including SystmOne, TeamNet, Word, Excel, Access, and other systems where necessary.

8. Freedomto Act and Autonomy

Expected to work on own initiative regarding own allocated work. The post holder will be required to manage own workload and willbe required to adjust priorities as required to meet continually changingservice needs. Work is discussed in regular supervision sessions and the postholder is responsible for deciding which issues need to be referred to Line manager. To ensure that relationships and own behaviour is managed in linewith the practice policies To always work with due regard for personal safety, adhering to Health& Safety requirements of the service.

Patients entrust and permit us to collect and retainsensitive information relating to their health and other matters, pertaining totheir care. They do so in confidence and have a right to expect all staff willrespect their privacy and maintain confidentiality at all times. It isessential that if, the legal requirements are to be met and the trust of ourpatients is to be retained that all staff protect patient information andprovide a confidential service.

12. CollaborativeWorking

All staff are to recognise the significance of collaborativeworking. Teamwork is essential in multidisciplinary environments. Effectivecommunication is essential and all staff must ensure they communicate in amanner which enables the sharing of information in an appropriate manner.

13. Security

The security of the practice is the responsibility of allpersonnel. Staff must ensure they remain vigilant at all times and report anysuspicious activity immediately to their line manager. Under no circumstancesare staff to share the codes for the door locks to anyone and are to ensurethat restricted areas remain effectively secured. Person Specification

Qualifications

Essential

MBBCH and MRCGP or Hospital Physician.

Experience

Essential

Experience of delivering patient care in a primary care, urgent or community setting. In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. Experience of multi-disciplinary working. Clear understanding of professional responsibility and accountability. Knowledge of current clinical evidence based practice.

Skills and Personal Qualities

Essential

Excellent communication and interpersonal skills including emotional intelligence to in order to deal effectively with complex and sensitive patient information, distressed or aggressive patients, or those with an impaired level of understanding. Ability to liaise and communicate effectively with all members of the multi-disciplinary team and other services. Ability to perform holistic assessment, critically analyse complex patient information or situations. Ability to critically analyse patient information or situations and make clinical judgements and decisions as to the best course of action. Strong IT skills.
