Care Planner Job, Temporary, Monthly renewal on a rolling basis.

Our client provides Extra Care Services and Housing to over 55-year-olds and is passionate about making a difference in the lives of the elderly within a residential setting.
Location: Near Gatwick - This is an on-site role
Role: Care Planner
Contract Type: Temporary (Agency)
Duration: 1 Month initially with a rolling contract
Working Hours: Monday to Friday, 9am–5pm, 35 hours per week
Pay Rate: Competitive

Key Responsibilities 

Writing Person Centered Care Plans.Reviewing the care plans and their implementation with the staff team.Working closely with the team to ensure quality of service.Writing risk assessments.Maintaining accurate records using Excel and other management systems.What you'll need to succeed 
Enhanced Adults DBS ideally on the update service.A minimum of 12 months relevant experience in Social Care or Sheltered Housing where you will have designed person centered care plans.Practical experience speaking to and working with residents to develop their care plans successfully.The ability to work with a current staff team to implement your care plans.What you'll get in return
