The maternity team at CDDFT are searching for a committed,dynamic and enthusiastic midwife to join our community team based in Chester LeStreet and around the historic city of Durham.

We are seeking a community midwife who is enthusiastic andcommitted to working in collaboration with women & birthing people,families and the multi-disciplinary team, supporting personalised choice andpromoting exemplary care in line with national transformation ambitions and ourtrust values.

Main duties of the job

We are looking for a passionate, committed band 6 midwife toprovide maternity care within our Derwentside community midwifery team at ChesterLe Street Hospital and around the local area. You will be responsible for theplanning, implementation and evaluation of individualised care plans whichensure delivery of holistic, research based care, prioritising communication,informed consent and relationship building. CDDFT community midwives arepassionate about facilitating antenatal and postnatal continuity of care andyou will be supported as an autonomous practitioner to plan your diary aroundyour caseload, ensuring that women and families are at the centre of everythingwe do.

Our team at Durham and CLS provide care to womenand families across a varied setting, including the beautiful city of Durham.Our community hospital works closely with our maternity units at the UniversityHospital North Durham and at Darlington Memorial.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Are you a registered midwife with a passion for communitycare?

We are looking for an enthusiastic and dedicated band 6Midwife who is keen to join and help further develop our community midwiferyteam based at Chester Le Street Community Hospital. We have so many new developmentopportunities on the horizon at CDDFT it is a very exciting time to be a partof our Maternity family

Our team of community midwives within Durham & CLSprovide care to women and families across a varied urban and rural area,including the historic cathedral city of Durham. We have a very diverse caseload, including vulnerable and isolated communities, providing opportunites tomake a real difference to the families in your care. We have a well establishedand well led team of midwives within the team, who you will find very welcomingand supportive. The team are assisted by the wider maternity team, including adedicated Community Matron and Matron for Public Health and Inequalities.

We are very keen to encourage community midwives with aninterest in providing care across the pregnancy and postnatal journey to applyto work with us to deliver outstanding community based Antenatal and PostnatalCare to the mothers and families in this area. We are also commited to relaunchingthe provision of our home birth service and would particularly welcomecommunity midwives with an interest in further developing this important areaof care. The focus of our dedicated community teams is patient centred andindividualised care, providing support and continuity throughout the antenataland postnatal period and promoting choice to families from across our diversecommunities.

We are looking for an individual with a passion for healthpromotion, as the role of community midwife at our trust includes workingwithin the UNICEF framework for Infant Feeding, and close working relationshipswith our specialist antenatal and newborn screening Team to deliver allelements of the National Screening programme.

You will be very well supported with a bespoke induction andtraining package by members of our specialist Practice Development Team whowill ensure your support as you transition to your new role both professionallyand personally. The successful candidate will be supported and encouraged toseek educational opportunities to further their Continuing ProfessionalDevelopment and career progression. In turn, you will have the opportunityto assist in supporting the development of our preceptorship, InternationallyEducated and student midwives.

All of our teams work under the care of a dedicated teamleader in each team, overseen by our Community Matron and the Matron for PublicHealth and Health Inequalities. Each team also has access to admin support andband 3 Maternity Support Workers who offering close support to each team. Youwill also be able to access personalised pastoral care via our specialistworkforce experience teams, including our retention lead midwife and the PMAservice.
