Job Summary/Job Purpose:1. Main duties of the job

DUTIES AND RESPONSIBILITIESLead pharmacist for the Prison healthcare unit. This will involve:1. Attendance at healthcare departmental and audit Monitor drug use and expenditure and provide reports to pharmacy management team. Providefurther information should advice be Responsible for providing a specialist clinical pharmacy service4. Provide recommendation to senior departmental medical and nursing staff on drug related Ensure adherence to national and local protocols and Identify and lead drug related research and audit Provide advice to clinical services manager for service development within Healthcare non medicalprescribing and other areas as Assess patients' compliance with medication and consider best treatment based on Undertake patient and/or relative care counselling using terminology suitable for To lead the development of medicines management within Prison Healthcare

Job responsibilities

Working Relationships and Communication Requirements of the Job1. To plan and organise pharmacy services to a clinical area, managing the activities of relevanttechnical and assistant staff to ensure adequate stock distribution and timely dispensing forindividual Communicate any unusual drugs or drug use to the procurement Ensure storage of medicines on Prison wings and in pharmacy department complies withnational legislation and local To analyse and review system one drug charts. This will include:- - Identifying and reportingadverse drug reactions and side effects. - Report Adverse Drug Reactions to the Committee ofSafety of Medicines. - Analyse and interpret individual patient laboratory data in order to provideTherapeutic Drug Monitoring, biochemical and haematological monitoring ensuring safety andefficiency of medicines. - To ensure drugs are prescribed in accordance with their product licenceor used appropriately when used outside of their licence. - Identify and interpret drug interactions.- Identifying and solving drug problems for individual patients. - Proactively makerecommendations to medical and nursing staff on medication issues. - Provide evidence basedrecommendations to influence prescribing. - Provide skills for and information on the correctpreparation and administration of injectable preparations and administration of enteralpreparations - To compile a patients medication history to facilitate the quality assurance of thepatients prescription Provide highly complex and potentially contentious information to all levels of medical andnursing staff, based on a combination of knowledge, interpretation of evidence and adaptability ofindividual patients. Order investigations and interpret/act on results as appropriate and within scope Make additional therapeutic interventions as appropriate to contribute to a patients holisticchronic disease management signpost on lifestyle issues such as smoking Identify and undertake training as appropriate to extend relevant skills and wider clinicalexpertise BP monitoring, spirometry, examination skills, point of care testing, flu If appropriate and if required for the role, to progress as a non-medical prescriber and prescribewithin clinical competencies in accordance with guidance and based on patient clinical and holisticneed.
