Main duties of the job

To provide advanced clinical and professional OccupationalTherapy/Physiotherapy to the Community Therapy Team/Community Hospital Team,and to provide support to the Integrated Community Care team leaders inensuring that the team contributes to the implementation of Southern Health ClinicalGovernance Strategies.

SouthernHealth provides a wide range of mental health, physical health and learningdisability services for people across Hampshire. Our aim is to work alongsidethe people we support and our health and care partners, to constantly improveand deliver the best possible care.

Gosport WarMemorial Hospital provides a range of inpatient and outpatient services for thelocal community. Our Community Physical Health Services provide physical healthservices across much of Hampshire both in the community and in our inpatientservices. We have a number of care pathways, based on different conditions(such as physiotherapy or respiratory) which we use to provide very tailoredtreatment for our service users.

The Responsibilities

If you are successful in your application to this post, youwill be expected to manage and deliver on the responsibilities outlined below as well as those not included here, call today to discuss further:

Undertake assessments of patients with diverseand complex conditions, providing a clinical diagnosis (where appropriate),development, and delivery of treatment programmes. Provide expert clinical advice and therapy topatients with a variety of complex conditions living in the community who areat risk of deteriorating health that may result in declining quality of life oravoidable hospital admission to secondary care To provide clinical OccupationalTherapy/Physiotherapy skills within the Community Therapy Team/ CommunityHospital Team(s), and all other provider organisations. Assess and treat a range of complex factorsrelating to the patients condition, diagnosis and prognosis To provide specialist intervention tailored tomeet the uniqueness of the individual.

To be successful in applying for this role you willneed:

Enhanced DBS Degree level qualification or equivalent To be registered with the Health and CareProfessions Council (HCPC) Experience of working as part of amultidisciplinary team NHS experience Rehabilitation experience Previous experience of working in a communitysetting
