Job summary

The Advanced Nurse Practitioner will be part of the clinical team providing face to face consultations in a primary care setting with the support of two GPs and two practice nurses.

You will work as an autonomous practitioner within the practice multidisciplinary team, managing undiagnosed problems and delivering care within your scope of practice to our patients.

The post holder will assist in developing protocols and guidelines within the practice and work with colleagues in ensuring the highest standards of care for all patients.

Main duties of the job

Manage patients presenting with a range of acute and chronic medical conditions Implement and evaluate individual specialist treatment plans for chronic disease patients Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Process and interpret pathology and other tests as required Maintain accurate clinical records Ensure SNOMED codes are used effectively Support patients in the use of their prescribed medication or over the counter medicines (within own scope of practice), previewing annually as required Contribute to practice targets (QoF, etc) complying with local and regional guidance Support the clinical team with all safeguarding matters, in accordance with local and national policies

We are part of South West Shropshire PCN who provide support in the form of FCPs, Pharmacists and Social Prescribers

Job description

Job responsibilities

ANP working alongside GP partners within a small friendly practice

Provide first point of contact care for patients presenting with undiagnosed problems, utilising history-taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient.

Recognise and work within own competence and professional code of conduct as regulated by the NMC and RCN General Practice Advanced Nurse Practitioner Competencies.

Undertake face-to-face, telephone and online consultations for urgent and routine problems including the management of patients with long term conditions.

Participate in all treatment and preventative healthcare services in the Practice, as delegated and agreed by supervising GPs.

Perform diagnostic physical examinations and treatment

Identify, signpost and refer patients at risk of developing long-term conditions, preventing adverse effects of patients' health.

Ensure continuity of care, arranging follow-up consultations or reviews as necessary.

Attend regular Practice clinical meetings.

Support clinical team with all safeguarding matters in accordance with local and national policies.

Recognise, assess and refer where appropriate, patients presenting with mental health problems.

Deliver integrated patient-centred care through appropriate working with the wider multi-disciplinary team and social care network.

Act on alerts and reminders during consultations (eg QoF, local enhanced services).

Assist Practice in achieving our clinical targets and objectives.

Apply infection-control measures within the Practice according to local and national guidelines.

Person Specification

Qualifications

Essential

ANP - MSc Advance Practice or equivalent. Independent Prescriber. Registered Nurse with Nursing and Midwifery Council. Meets the standards for registered ANP working at advanced level.

Desirable

Experience of using Emis Ability to work flexible hours when required Minor illness qualification.

Experience

Essential

Experience of practice within the four pillars of clinical practice; leadership and management, education and research with demonstration of core capabilities and area specific clinical competencies. Job plan that demonstrates advanced nursing practice and has equity with peers working at this level. Experience of prescribing and undertaking medication reviews. Experience of working in a primary care environment.

Desirable

Experience of working as a practice nurse or community nurse.
