The applicant will have knowledge and experience ofworking with patients with a diagnosis of stroke and with patients with complexdysphagia and communication difficulties. They will be confident withindependent working and have experience of MDT working. The postholder will begiven a service induction including training on patient related IT processesand systems.
HWHAC have seven communityhospitals/recovery units across Worcestershire. They provide a range ofinpatient and outpatient services and support people to recovery from surgeryor illness when they don't require acute care but neither can they be treatedat home. They are a key part of the local healthcare system.
Main duties of the job
Role Details:
To assess, treat and manage own caseload ofpatients and to maintain accurate clinical records
To devise and deliver specialised programmes ofcare and communicate treatment details to patients, carers, and members of theMDT
To formulate accurate prognosis and recommend thebest course of intervention, developing comprehensive discharge plans
To monitor and evaluate the effectiveness of Speech& Language Therapy interventions and modify as appropriate
To follow and contribute to the development ofdepartmental policies/guidelines and to participate in the development ofinnovations in service delivery taking account of clinical governance
To beresponsible for maintaining own competency and self-developments
About us
The Successful Candidate Will:
Hold a Degree in Speech & Language Therapy orequivalent
Be HCPC registered
Have previous NHS experience
Be dysphagia competent
Practicein accordance with the professional, ethical, and legal codes of the Health& Care Professions Council (HCPC), and Trust and NHS protocols andguidelines
In addition to experience, allQualified roles will be required to provide evidence of:

Registration with the relevant Registration Body if applicable
