Job summary

An exciting opportunity has become available within the Adult Congenital Cardiac Specialist Nurse team.

We are looking for a motivated, enthusiastic, and dynamic individual who will be responsible for a managed caseload working closely within the adult congenital team and who can ensure the consistent delivery of high-quality patient centred care.

A good understanding of current issues in healthcare both locally and nationally, involvement in improving patient safety, risk and waste reduction and the development of others and general support for peers, junior and senior colleagues is expected.

Candidates should have current NMC registration and experience within cardiology. Potential candidates are advised to read the job specification prior to submitting an application.

Main duties of the job

The job holder will be responsible for supporting patients with congenital cardiac conditions and their family within various settings including Outpatients (Surgical & Cardiology), cardiology wards, and regional outreach clinics across the Yorkshire and the Humber region. The successful candidate will be expected to work with the multi-disciplinary team and will contribute to data collection and audit.

The successful candidate will be self-motivated, able to work autonomously and have excellent communication and organisational skills. They must work with The Leeds Way and Trust values at the centre of their care, for both patients and colleagues. They must be able to demonstrate current and on-going personal development and evidence of their contribution to change management/service development within their current post.

Job description

Job responsibilities

The Jobholder is responsible for a managed caseload working closely within the paediatric congenital nurse team. The jobholder carries responsibility for the assessment of care needs, the development, implementation, and evaluation of programmes of care. The jobholder will provide specialist nursing care and advice to this specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.

The post holder would be preferably from the background of cardiology and will support the Congenital Cardiology team in the Outpatients clinics, Wards & via a telephone advice line providing direct patient care & advice for those children with congenital heart disease and their families. They will support the education of the MDT team specifically about congenital heart disease. They will contribute to data collection and audit.

The post holder will work within a team of adult congenital cardiac specialist nurses who support the Congenital Cardiac Services.
