Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The Clinical Effectiveness programme in SE London (CESEL) provides a systematic evidence-based approach to quality improvement across SE London GP practice and Primary Care Networks (PCNs), to help to improve health outcomes for local residents across SEL and further develop a successful Integrated Care System (ICS).
Built on successful models already established, the function brings population health management and learning together, to form the basis for personal, organisational and system development, and continuous improvement in outcomes.
The CESEL programme Clinical Lead/s provide clinical leadership across the programme and CESEL team, which includes providing mentorship to the place-based Clinical Leads, who delivery local resources and support ( Clinical Effectiveness Guides, education) to primary care staff.
They also lead the ongoing development of the clinical effectiveness functions, working flexibly to deliver ICS and local priorities. This also includes an ambition to develop a system-wide quality improvement function that reaches beyond general practice.
Please note; CESEL are seeking 2 clinical leads to provide a total of 8 clinical sessions (4 sessions each).
Main duties of the job
o Drive forward the development, implementation and delivery of a programme of clinical Lead and support a multi-professional, cross-provider partnership teamo Provide expertise and leadership in shaping, delivery and maintaining a systematic approach to quality improvement, ensuring national and local best practice is embedded and delivered via a range proven methods and resources o Inform and steer the local development and utilisation of quality improvement methods and toolso Leads and supports the place-based clinical leaders to understand, describe and navigate systems challenges, to inform an effective approach to facilitating quality improvements across primary careo Inform, design and lead the delivery of education and training for frontline clinical staff.
