Job summary

We have an exciting opportunity to recruit a Senior Physiotherapist We are seeking highly motivated, committed and experienced Senior Physiotherapist's to support in the development and delivery of our rapid response service within Furness. You will have the unique opportunity to influence change as we integrate physiotherapy within the service and community care.

Our speciality is admission avoidance of which therapy is a key component of the service delivery. Our aim is to support and treat suitable patients in the community, with a range of escalated health and social care needs. Rapid Response's aims are preventing hospital admission and support early discharge from hospital.

The Rapid Response Team is currently affirming and embedding the urgent care model. The urgent community 2 hour response target and virtual wards are very much part of our service specification and builds on the visions outlined in the NHS Long Term plan. Our virtual ward beds are a mix of Acute Medical and Frailty virtual ward beds to provide hospital at home.

Our team is a multidisciplinary team who provide who collectively integrate skills and share knowledge which includes performing non traditional roles and some cross boundary working. In addition to this we work closely with other Community Teams such as the Respiratory service, the UCR Hub, District Nurses and many more.

Main duties of the job

As a Senior Band 6 Therapist you will:

Deliver high-quality urgent care within your gift

Have strong clinical decision -making skills

With your expertise and skill, strengthen the care plans delivered from your senior triage and assessments

As part of a multidisciplinary team assess and treat conditions within the patient's own home.

Plan and implement person centre care especially focussing on frailty syndromes and Respiratory conditions, including chest physio.

Help to improve outcomes for patients as well as the patient experience both within the service and wider organisation

Work collectively to improve system working across acute, community and social care settings

Share your knowledge and skill

Utilise best practice

be able to work autonomously

Delegate work

be open and engaging for some cross boundary working ( training and competency assessment will be offered)

Demonstrate an understanding and appreciation for your own role and the different roles within the team and how they work in collaboration.

Undertake timely assessments.

Ensure assessments are of a high standard, with planning, implementation and evaluation of care plans accordingly.

About us

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Person Specification

Qualification/Education

Essential

Honours Degree or equivalent in Physiotherapy approved by the Chartered Society of Physiotherapy/ Health and Care Professions Council State registration with Health and Care Professionals Council HCPC Evidence of relevant post graduate specialist clinical education

Desirable

Membership of the Chartered Society of Physiotherapy Post graduate training or experience to post graduate diploma level or equivalent. Accredited Practice Placement Educator's qualification (or willingness to attain within 2 years)

Experience

Essential

Relevant post registration experience as a physiotherapist to achieve knowledge and skills applicable to this post. Comprehensive rotational experience as a therapist in the core competencies within Physiotherapy for example: musculo-skeletal, neurology and cardio- respiratory care Experience of working within a team Experience as an autonomous practitioner.

Desirable

Experience of research or audit. Experience of working as a Therapist of your profession within the community setting such as admission avoidance or rehabilitation Experience of working with frail patients, and knowledge of frailty tools and frailty pathways. Experience of working with patients with Respiratory Conditions / Chest Physiotherapy Experience as a supervisor of Basic OT/physio and/or OT/physio support staff and/or OT/physio students Experience of assessment and prescription of aids and minor adaptations.

Sills, Ability, Knowledge

Essential

Transferable knowledge and skills at basic practitioner level with aptitude to develop to specialist OT/Physiotherapist level. Ability to analyse professional and ethical issues Assessment, observation, evaluation and reporting skills Knowledge of current best practice in OT/Physiotherapy. Ability to work under pressure and recognise when support is required. Written and oral communication skills in English sufficient to communicate effectively with patients to gain consent and explain treatment interventions and gain information required for the patient's medical/social history relevant to OT/Physio. good interpersonal skills application and understanding of polices and procedures, clinical governance, health and safety, professional conduct, confidentiality Good team working Evidence of Continuing professional development

Desirable

Training and facilitation skills/ Supervisory skills Willingness to cross boundary work and undertake skills such as basic pressure sore prevention, clinical observation taking and recording in relation to NEWS 2.

personal qualities

Essential

Ability to meet the physical requirements of the post. Ability to participate in manual handling tasks and therapeutic handling, using skills developed through practice, including dexterity, co-ordination and physical effort, during OT/Physio interventions and the use of specialist tools and materials and installation of equipment. Willingness to work flexibly to meet unexpected and unusual circumstances related to providing direct patient care. Ability to travel to provide assessment and treatment to patients in the community and attend meetings.
