The post is a senior clinical role responsible for ensuring clinical effectiveness and coordinating services for individuals with complex learning disabilities. It involves understanding and planning for their health needs, covering both physical and mental health, managing cases, and providing person-centered care. The role also includes collaborating with other agencies to ensure reasonable adjustments are made.

This role may not be eligible for sponsorship under the Skilled Worker route, please refer to the Direct Gov website for more information with regards to eligibility.

Main duties of the job

As part of the multidisciplinary team you will be responsible for assessing, planning and managing risk of individual's care needs including physical, mental health and behaviour that challenges services.

Case management and care coordination for those that are fully health funded. Liaising with commissioners in conference regarding the identified health needs and how they are being addressed.Speaking up for individuals with a learning disability and ensuring reasonable adjustments are made by generic services to allow the service user to have equitable health care by the most appropriate service.Liaising with other agencies, providers and families in partnership with the individual and to keep in mind the safeguarding process.Ensuring consideration of capacity, restrictive practice in line with best interest and court of protection processes.To be an autonomous practitioner and be able to prioritise workloads to meet the demands of the service and the wider Livewell SW objectives.

Job responsibilities

ClinicalResponsibilities

1.Participatingin the undertaking of health needs reviews and health assessments to identifyneeds and plan care accordingly.

2.Toidentify, plan and manage risk of an individuals care needs, relating tophysical health, behavioural and or mental health.

3.Tobe responsible for assessing, planning and making recommendations for futurecare in partnership with patient, carer and multi-professional agenciesincluding case conferences for reviewing the packages of care and careco-coordinating as appropriate.

4.Toliaise with Primary and Secondary care agencies to ensure better healthoutcomes for individuals.

5.Towork within the Mental Capacity Act and evidence best interest decision making.

6.Tomaintain effective and appropriate communication networks with members of themulti-disciplinary team, the independent sector homes and all stakeholderagencies and organisations to ensure that the individual care needs can be met.

7.Tocontribute to multi-disciplinary team specialist assessments.

8.Abilityto work as an autonomous practitioner, planning, prioritising and organisingworkload according to the needs of the service.

9.Toprovide any relevant information and explanation to clients and carers at alevel commensurate with their ability to understand using total communication.

10.Toprovide evidence-based advice and guidance to care staff on the management ofhealth needs to patients and signpost to other services.

11.Toact as a supervisor and/or assessor for students demonstrating high standardsof practice and professional conduct.

12.Toundertake health promotion/education whenever possible to all adults with LDand also to contribute to planned health promotion activities.

13.Towork with all other agencies with regards to the safe guarding process.

ManagerialResponsibilities

ProfessionalResponsibilities

1.Tomaintain a high professional standard of conduct at all times in accordancewith NMC/ professional bodies code of professional conduct. Maintain an active status on NMC/professional bodies register.

2.Tomaintain own re-validation portfolio.

3.Assumeresponsibility for own professional development and personal knowledge.Undertake and contribute to research where appropriate.

4.Monitorown performance against agreed objectives through the process of appraisal andpersonal development plans and professional body regulations.

5.Maintainprofessional expertise by arranging and attending meetings, study days andmandatory training and service training.

6.Planand participate in the supervision and provision of experience for all nurselearners and other personnel as appropriate.

7.Participatein regular line management, caseload management and clinical supervision.

8.Todeliver appropriate training and subsequent support to other staff, studentsand outside agencies when required.

5. Communications and relationships

Thepost holder will be required to work within the MDT function of the CLDT andmay need to be part of wider MDT groups relating to client need.

Thepost holder will liaise between other health, social and educational servicesin assessing, planning and reviewing client care needs/service provision.

Thepost holder will also work with other voluntary and secondary care providers.

Person Specification

Qualifications

Essential

DipHE Nursing and or Degree Professional Registration. 3 years Post registration experience. Evidence of continuing professional development. Evidence of managing a caseload of complex individuals.

Desirable

Mentorship Certificate. Leadership qualification/knowledge

Knowledge

Essential

Knowledge of Continuing Health Care (CHC) processes Knowledge of the Mental Health Act. Able to demonstrate understanding and competency in assessment, care planning and review. Good knowledge and understanding of the Mental Capacity Act, Deprivation of Liberty and reasonable adjustments. A good understanding of the safeguarding adult process and relevant issues. Able to problem solve and be creative in care planning

Experience

Essential

Working in multidisciplinary teams and across health and social care. Managing a complex caseload of individuals who pose a risk to themselves and society. Experience of working across organisational and inter-agency boundaries. Experience of working with people with learning disabilities / mental health issues whose behaviour poses a risk.

Desirable

Experience in mentoring students. Working in close contact with the ICB and local commissioners

Additional Requirements

Essential

Full driving license is required as the post holder will be travelling out of area. The ability to be away from home overnight is an integral part of the role. Able to problem solve and be creative in care planning.
