The BSW Specialist Community Perinatal Mental Health Service is looking for a Care Co-ordinator to work within our team covering the south of Wiltshire.

The team is multidisciplinary, providing support for women who experience serious mental health problems or present with a high risk of relapse during pregnancy and the postnatal period. We offer assessment and treatment aimed at treating mental ill health and enhancing the mother infant relationship, where mental health needs have impacted upon this. We provide multi agency training in perinatal mental health to other professionals and agencies working with women in the perinatal period in addition to offering consultation and advice. Currently, the team is able to work alongside women and their families from pregnancy until their infants first birthday.

The Specialist Community Perinatal Team works across BaNES, Swindon and Wiltshire. We have a hub and spoke model - the hub is in Chippenham, Wiltshire. The spokes are in Paulton (BaNES), Swindon and Salisbury.

This post is suitable for experienced and professionally registered Nurses, Occupational Therapists or Social Workers. You must have experience of working with women in the perinatal period or a good understanding of the specific mental health issues that can affect these women.

Main duties of the job

Perinatal Mental Health is a rapidly expanding area with significant investment being made. This means that there will be high quality training and potential opportunities for career development for the successful candidates.

You will be required to provide assessment and care co-ordination for an allocated caseload of service users with perinatal mental health needs. This will include key tasks of undertaking high quality assessments, developing care plans, delivering and reviewing therapeutic interventions. You will act as a supervisor and role model to others. You will also be required to act as a mentor to students, cover duty on a rotational basis and provide training to teams both within and outside of AWP. Applicants will be expected to demonstrate a high level of decision making skills and to work independently. They must also have a good understanding of safeguarding.

Access to your own transport for work use is essential as public transport is unreliable in parts of the BSW area.

Person Specification

Education and Qualification

Essential

Registered as an OT, SW or RMN (parts 3 or 13 of NMC register) Formal post-graduate diploma level qualification in a relevant field

Experience and Knowledge

Essential

Significant post-qualifying clinical experience obtained in a variety of settings Experience of working with women experiencing perinatal mental health needs/knowledge of demographic risk factors for this client group Clear record of ongoing continuing professional development Demonstrates experience as a Care coordinator or equivalent role of complex cases Demonstrates a detailed understanding in the theory and practice of Recovery Principles

Desirable

Experience or delivery of training / teaching within MDT/external organisations
