Home First MDT Community Hub

We are seeking experienced Social Workers who thrive in a fast-paced environment. The ideal candidate will share the vision of 'Home First' & the Reablement ethos: where home is best and patients are supported to recover at home wherever possible. The Home First Hub is person-centred and strengths based: HF team works together to maximise the opportunities to discharge people , preventing any unnecessary delay. We are fortunate to have representatives from Occupational Therapy, Physiotherapy, Nursing, Mental Health, Housing, Public Health & the Third Sector all contributing to achieving the best outcomes for our service users.

You will be a dynamic & forward-thinking social worker with a strong value base. You will be a confident communicator who enjoys promoting the social perspective within a multidisciplinary environment. You will enjoy working closely with a wide range of professionals and be able to adopt a flexible, creative and solution-focused approach. Based at St James' House, Salford which has excellent transport links & free parking close to the office. Please note that access to transport is required

Main duties of the job

As a Social Worker in the Home First Community Hub, some of your responsibilities will include:

Adopting a solution-focused, strengths based ethos.Working to the principle that 'Home' is the discharge destination that is considered 'First' Utilising multidisciplinary assessment skills supported by sound reasoning to facilitate timely hospital discharge to the most appropriate environment.Responsibility for triaging referrals received in the Home First Hub. This will involve liaison with patients, carers, ward staff and colleagues to identify the most appropriate discharge pathway .Working collaboratively with partner organisations on a daily basis to develop person-centred rehabilitation and reablement care and support plans .Effectively triaging and prioritising workload and adjusting daily work plans in response to system-wide pressures and escalations.Proactively optimising the usage of our intermediate care beds with an awareness of capacity on a daily. Providing coordination for the team on a rotational basis to allocate tasks and lead MDT discussions Carrying out Care Act (2014) assessments

Promoting patients' wellbeing throughout their journey from hospital to home, or other appropriate setting

Knowledge of related legislation, policy, and frameworks

Knowledge of Safeguarding processes and procedures

Qualifications

Essential

Possession of recognised Social Work Qualification: SW Degree, CQSW, CSS, Dip SW or equivalent Social Work England Registration

Experience

Essential

Clearly evidence (in your practice) an understanding of the principles and values underlying the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation and guidance such as Continuing Healthcare etc Clearly evidence your ability to develop effective working realtionships with service users and their carers (in your practice) and show evidence of good assessment practice. You must clearly evidence that you have the ability to maintain accurate records to communicate complex information clearly and concisely both verbally and in written form You must clearly evidence that you have a clear understanding of the social work role in a multi-disciplinary setting. Demonstrate the ability to develop effective working relationships with other professionals and colleagues. Possess skills of negotiation, mediation and de-escalation - seeking constructive approaches to problem solving. Experience of working with adults/older people. Have a good standard of computer skills.

Desirable

You have additional qualifications; Best Interest Assessors; AMHP; Practice Educator; Post Qualifying courses or other professional development qualifications. You can evidence an innovative approach to the development of new methods of working to support people in the community.

You must clearly evidence your understanding of the integration agenda and the promotion of independence (enablement/strength based practice) model of health and social care Demonstrate a thorough understanding of personalisation, person centred and strength based practice

Desirable

You can show knowledge of relevant Welfare Rights Legislation. You can show knowledge of the recent Government Guidelines.
