Bridge and Monkseaton Medical Practice is an innovative, friendly and hard working practice. We are enthusiastic training Practices that host GPR and F2 doctors and medical students. We are based in two locations, Monkseaton and Shiremoor.

We are looking to recruit a caring and enthusiastic Pharmacist to join our busy multi-disciplinary team. Experience preferred. We would require the successful candidate to be able to work independently for 18/27 hours working 8am - 6pm over 2/3 days.

Main duties of the job

To promote safe, rational, high quality and cost-effective use ofmedicines by working closely with all practice staff and the wider primaryhealthcare team

To encourage a safe, responsible and respectful attitude to prescribingand medicines use within the practice

To support the lead Pharmacist(s) and Nursing teams in the management ofpatients with Long Term Conditions.

To work within your competencies to offer patients the optimummanagement of their medical condition. To recognise the limits of yourexpertise and to communicate with other members of the PHCT. You will seekadvice and support from your colleagues when necessary.

About us

We are a 2 site Practice, Monkseaton Medical Centre based in Whitley Bay and Bridge Medical located in Shiremoor. We are led by 3 GP partners. We employ a Management team, 9 salaried GPs, 2 Practice Nurses, 3 Health Care Assistants, 2 Pharmacists and a Pharmacy technician. We have a navigator system that has been developed specifically to the Practices vision and this helps with continuity of patient care. We are very focused on team development holding monthly staff meetings and holding 4 Time In Sessions per year. We hold socialised events and enjoy being part of a large work family.

Job responsibilities

Quality ofprescribing

Undertake the management of patients with Long Term Conditions (LTC) including annual review of their condition, medication and blood tests. Support the diabetes lead GP, pharmacy and nursing teams with management of patients with Diabetes Review and file annual LTC blood tests for all patients in the practice Support best clinical management of LTC in line with national and local guidelines and protocols. Recognise when patients would benefit from a change in medication and work discuss with prescribers to implement and facilitate this. Ensure optimum management and treatment of hypertension, heart failure, AF, CVA, PAD , CVD, diabetes and CKD Calculate QRisk and using shared decision making with patients prescribe statins where appropriate Adjust levothyroxine doses in line with TFTs Medication review for all patients on repeat medication and maintenance of the medication review system Provide medication review for patients in care homes Review list of medicines for newly registered patients and add to their medication record as appropriate and ensure patients are entered into any monitoring systems, medication review dates added Ensure annual monitoring tests are performed and reviewed where appropriate, ACE-I/A2RA, amiodarone, digoxin, lipid lowering drugs, warfarin, NOACs, antipsychotics, antiepileptics, antidepressants, lithium and DMARDS. Provide medicines reconciliation following discharge from hospital. Review discharge information and update patients repeat medication list as necessary. Ensure prescribing in the practice is evidence based and follows local and national guidance. Raise awareness of new national ( NICE) and local guidance relating to medicines use and provide information to prescribers which is summarised and made specific to the particular practice ( in terms of IT considerations, formulary drugs) Support the practice to improve prescribing quality and performance against national and local targets. Proactively provide prescribing advice to prescribers and clinical staff in the practice regarding changes in drug safety information, discontinued products, and changes in licensing. Be aware of CAS reporting systems in the practice. Proactively provide information to prescribers regarding news story items in the media and provide sufficient detail to be enable practitioners to critically appraise the information provided in the press and put in context Provide summary information on new drug information as needed

Cost-effectiveuse of medicines

Provide expert advice to promote Medicines Optimisation within the practice. Support the practice to achieve QOF and improve performance. Implement and support Repeat Dispensing and promote Electronic Prescribing. Liaise with local community pharmacists and act as a point of contact regarding any prescription queries, amendments or appropriate switches for out of stock medicines. Effectively manage your time in relation to this work and avoid being reactive. Act as a point of contact for prescribers and pharmacies for patients using monitored dosage systems and repeat dispensing relating to any changes in drug treatment Regularly monitor and review high risk medicines and shared care prescribing Monitor use of special drugs and give advice regarding alternatives to unlicensed medicines Monitor prescribing of red drugs Complete SIRMS reports. Manage robust systems for instalment/weekly dispensing of drugs liable to misuse or overuse Work with the practice pharmacy team to implement the DES for safe prescribing of DMARDs, ensure regular monitoring is undertaken and bloods results are within acceptable parameters before prescribing Review out-patient letters which include changes to drug treatment and make changes to patients repeat medication list as appropriate (or query prescribing if not in accordance with Formulary) Review outpatient discharge recommendations and issue prescriptions as requested Report any secondary care recommendations to prescribe non-formulary or red drugs to the local medicines management team and complete SIRMS reports as appropriate. Maintain the practice formulary (and other formularies) on the computer. Provide training to new reception staff regarding issuing prescriptions Contribute to the training of other healthcare professionals ( new GPs, GP registrars, students) and reception staff regarding all aspects of medicines management and prescribing Triage queries from reception staff regarding patients making unusual requests for past medication, overuse of medication and refer to GP as necessary Prepare and present quarterly prescribing meetings to clinical staff Review quarterly prescribing data and prescribing budget spend and provide any necessary information to prescribers Assist with prescribing audits arising from safety concerns or good prescribing practice Attend Clinical meetings at the practice including SEA meeting and others as requested.

MedicinesOptimisation

Provide information and guidance to patients to improve adherence with medicines taking. Promote and support the use of shared decision making. Be involved in developing and reviewing of practice protocols/guidelines which include use of medicines Respond to any needs of the practice regarding improving safety of prescribing, critical incidents and adverse drug reactions Report adverse reactions to drugs via the Yellow Card reporting system Monitor use of drugs for patients who may be overusing or misusing drugs and alert prescribers as necessary Respond to queries from GPs and other clinical staff and contact patients where necessary Respond to queries from receptionists when they send tasks regarding any medicine-related matters and contact patients where necessary Person Specification
