We have a newly developed post of Deputy Manager & Senior Clinician for the Buckinghamshire Primary Care Mental Health Hub (Bucks PCMHH). Bucks PCMHH is a new and innovative service, which has been set up as part of delivering the Community Mental Health Framework (CMHF) objectives of providing care and support to people in their own communities. The team brings together a range of health care professionals and support staff, to provide care and treatment to patients in their own communities.

We aim to deliver more proactive and responsive care to reduce
unnecessary hospital admissions, secondary mental health service referrals, or repeat GP appointments wherever possible. PCMHH Teams aim to offer a collaborative approach to deliver care for people with multiple long-term conditions by providing better, local and faster access to services for patients at an earlier point in the care journey. PCMHH Teams offer careful, holistic assessments based on a biopsychosocial care model.

Main duties of the job

You will work as the lead clinician for the Bucks Primary Care Mental Health Hub, supporting the Team Manager in supporting the team, whilst also working directly with clients.

The team comprises of mental health professionals, Peer Support Workers and 3rd sector partners who work together to provide short-term interventions for people experiencing mental health issues who need more than can be offered by the GP surgery and IAPT service, but not requiring the input of secondary mental health services. Support is offered through group and individual interventions.

You will support the team to focus services on local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve.

Advanced understanding of the mental health needs of adults. All new starters have a 6 month probationary period.
