We are looking to recruit Mental Health Practitioners to work in the CMHT Stockport West


The East CMHT is a multi disciplinary team with staff from a range of professions including, mental health nurses, occupational therapists, social workers and support workers. The team has dedicated Consultant Psychiatrist, and full admin support. We have access to Psychology and a newly commissioned personality disorder pathway and we have recently recruited a TAPPs and Mental Health And Wellbeing Practitioners to work alongside the CMHT to broaden our service for the people in Stockport.

Main duties of the job

To act as the care coordinator and co-worker responsible for the overall care of an identified group of clients, being part of a service that is committed to recovery and social inclusion.

To provide supervision to an identified group of staff and students on placement with the team.

Detailed job description and main responsibilities
  • Undertake the full range of casework for people with moderate to severe mental health needs. Adopting a recovery and social inclusion model that encompasses non health and social care services to meet service user needs.
  • Undertake assessment to the need for CMHT care and treatment under the Care Act 2014 and Care Programme Approach
  • Provide appropriate cover in the absence of the team manager / deputy, as necessary.
  • Implementation and evaluation of care.
  • Develop care plans for client / patients and undertake clinical activities as appropriate, ensuring a high standard of care.
  • Ensure all appropriate client / patient and departmental records are maintained to the required standard and in line with CPA policy.
  • Work with the wider multi-disciplinary team to enhance the standard of care client / patients receive.
  • Work within the framework of the Care Plan Approach acting as care coordinator as required.
  • Participate in risk assessment as care coordinator and formulation of risk management plans.
  • Participate in staff inductions to develop the skills and knowledge of newly qualified nursing staff.
  • If RMN to participate in the administration of Depot medication according to the Drug Prescribing Policy, and administer all other medication in accordance with agreed procedures.
  • Take a key role in communication within the multi-disciplinary team to ensure continuity of care.
  • Promote and adhere to the Trust's values statement.
  • Demonstrate high standards of Health and Social Care needs and be accountable for one's own practice.
  • Provide supervision to staff in the team according to the trust policy and standards.
  • Supervise and assess students during placements, ensuring appropriate advice and support is given.
  • Attend team meetings and care planning meetings as required , ensuring follow up action is taken.
  • Participate in the development of staff within the team.
  • Communicate effectively with all staff, clients / patients and carers in accordance with Trust / Directorate standards to promote high quality of care.
  • Ensure client / patient confidentiality at all times.
  • Take a keen interest o=in own personal professional development. Attending adequate training to ensure issues relating to service users and their families are assessed and addressed in relation to the policy, safeguarding adults.

Person specification Education / Qualifications Essential criteria
  • Professional qualification Social Work / Mental Health Nursing / Occupational Therapy
  • Registration with HCPC / NMC
  • Experience in community mental health work
  • Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate
Desirable criteria
  • Evidence of post-basic qualification and training
  • PSI training
  • MSLAP or equivalent Mentor training
  • AMHP
Experience Essential criteria
  • Knowledge of CPA policy
  • Experience of supervision
Desirable criteria
  • Experience of leading a team / acting up
Knowledge Essential criteria
  • Experience in working with people with moderate to severe mental health needs
  • Extensive knowledge / experience in social inclusion and recovery model of care
  • Knowledge of mental health legislation
  • Experience of MDT working
