This service recognises the need of the whole person and is given in co-operation with other agencies.

Main duties of the job

Working for St David's you will become part of a much wider team all working collaboratively to ensure the best care for our patients. Our mission is to provide and continue to develop a free and comprehensive palliative care service of excellence for people facing a progressive life-limiting illness which is no longer curable.

As part of the Inpatient Unit you will benefit from a salary in line with Agenda for Change pay scales and will enjoy a generous annual leave entitlement. We can offer the continuation of the NHS pension scheme if already a member as well as honouring any additional annual leave entitlements accrued as part of previous NHS service. You will have access to free on-site parking at both the inpatient and any of our office or training locations.

Job description

Job responsibilities

Key Tasks:

work in accordance with the policies and procedures of St. Davids Hospice Care and the Healthcare Inspectorate Wales.

maintain confidentiality at all times.

ensure patients comfort and dignity at all times.

work alongside patients and nurses, whilst also recognising their need for privacy.

perform basic nursing procedures in accordance with St Davids Care Plan and document all intervention.

report any changes in the patients condition to the nurse in charge of the shift.

help with patients personal hygiene and toilet needs as appropriate.

maintain a safe environment for self, patients and carers at all times.

be familiar with the Care Decisions Tool for the last days of life.

General Responsibilities:

recognise and accept the limits of personal knowledge and responsibility.

be aware of ones own responsibilities towards Health and Safety Regulations and all SDHC policies.

verbally report any accident or untoward incident as soon as possible followed by written details.

be aware of hospice policies and procedures.

undertake any training identified as necessary by SDHC.

participate in clinical supervision.

observe a No Smoking policy whilst on duty.

observe a No Sleeping on duty policy.

contact the nurse in charge in the event of any problems occurring during the shift or if their condition causes concern or if the patient dies.

a verbal handover from clinical team prior to shift commencement.

be aware of the risk assessment for each patient and act accordingly.

Qualifications

Essential

- Possess a Diploma / NVQ Level 3 in Health & Social Care or equivalent - Excellent communication, listening and interpersonal skills - Experience of working with palliative care patients - Demonstrate desire to further development of palliative care skills - To have an open, caring and inclusive manner - Can understand the principles and values of care - Can maintain safety at work - Has an understanding and awareness of safeguarding requirements
